                                       Health care Challenges in Africa
        Africa faces a burden of communicable and non-communicable diseases. On March 23, 2014, the World Health Organization (WHO) reported cases of Ebola Virus Disease in the forested rural area of Southeastern Guinea. A research done by Olu, Petu, Ovberedio & Muhongerwa, 2017 states that the 2014/2015 Ebola Virus Disease (EVD) outbreak in West Africa infected more than 28,000 persons out of which 11,000 died. At the height of this outbreak, a considerable load of cases and their contacts overwhelmed the response capacity of the principally affected countries namely Guinea, Liberia, and Sierra Leone. When this outbreak happened, the affected states didn’t have experience or tools to identify cases and limit the spread of the disease rapidly. Cost-effective interventions and medical access would have reduced the number of people infected during the epidemic/outbreak, but coverage is too low due to the health care system. These challenges relate to the leadership in Africa countries, health workforce, and medical products.
Challenges and the way forward
      All countries suffer from problems of coordination among hospitals and community-based services (Knickman & Knover, 2015, Chapter 4). One challenge is accessing, which is still the greatest challenge to health delivery in Africa. Fewer than 50% of Africans have access to modern health care facilities. Many African countries spend less than 10% of their GDP on healthcare. Secondly, shortages of trained health care professionals from Africa because many of them prefer to live in places like the United States and Europe. There is also the increase in communicable and non-communicable disease such as AIDS, malaria, hypertension, which are increasing in the middle-class and the poor increases.  I think the government is responsible for ensuring that everyone has access to healthcare. However, I don’t believe that healthcare is a public good that is the sole responsibility of the government in Africa. There should be an opportunity for entrepreneurs to enter the health delivery space in Africa to provide healthcare to the middle class and the working poor.  African Counties need to embrace technology to close health care gap and private-public partnership in the health care system. In 2007, Becton, Dickinson, and Company (BD) and the US President’s Emergency Plan for AIDS Relief (PEPFAR) entered into a public-private sector partnership focused on laboratory-system strengthening in sub-Saharan Africa (Cohen, 2016). This partnership is now known as “Labs for Life” was formed to help the people in low resource countries in Africa living with AIDS access to antiretroviral therapy.
According to Hader (2016),
Public-private partnerships (PPPs) align public and private needs around mutual goals to move vital projects forward. When PPPs work to strengthen the critical link in the healthcare system, such as laboratory networks, as demonstrated in this supplement by authors from the International Laboratory Branch at the Centers for Disease Control and Prevention (CDC), in-country officials from the respective CDC and Ministries of Health, implementing partners, and Becton, Dickinson, and Company (BD), they significantly change the landscape of healthcare and patient outcomes.
 
     Cognizant of the challenges mentioned above, the 46 Ministers of Health from the African Region adopted and signed the Ouagadougou Declaration that proposes ways of addressing health system challenges. The Ouagadougou Declaration urges Member States to update their national health policies and plans according to the primary health care (PHC) approach; promote inter-sectorial collaboration and public-private partnership to address broad determinants of health; improve health workforce production and retention; set up mechanisms for increasing availability and accessibility of essential medicines, health technologies and infrastructure; strengthen health information systems; develop and implement strategic health financing policies and plans; promote health awareness and build behavioral change capacities among communities.
Summary
        Efficient health interventions are available to help with the health care problems in Africa. Unfortunately, health systems are too weak to adequately and equitably deliver those interventions to people who need them. As nurses, we need to reach out to the health care teams and organizations in Africa to speak up for the population. Join International health organizations and write letters to the policymakers in countries in Africa. Let our voice be heard through international media to help curtail this problem. We need to make it a global problem and advocate for this population so they can have good health care and access to medical care by involving policymakers and legislators in their respective countries.

The Diagnostic and Statistical Manual of Mental Disorders (DSM) was originally developed in 1952 to help doctors and counselors across the country standardize the classifications of mental disorders in the American population. Throughout the intervening years, the DSM has gone through several revisions, establishing specific diagnostic criteria for each disorder listed, and revising disorders as more was understood about their origins, symptoms, and treatments. The DSM III (1980) marks the point when substance use disorders were moved to a category of their own, separate from personality disorders. Today, the current DSM lists the diagnostic criteria for substance use disorders.
While these standardized diagnostic practices have been in use for more than sixty years, there are some pros and cons in the use of the DSM diagnoses of substance use disorders in the assessment of clients.

For this discussion, you will take on the role of the private psychologist being asked to re-evaluate a client. Carefully review the ABS 300 Week One Assessment Scenario. In your initial post, write a rationale for either agreeing or disagreeing with the request to re-evaluate the client based on the information available. Explain which theoretical assumptions about psychological testing and assessment support your decision. Discuss the ethical responsibilities a psychologist must consider if choosing to use psychological assessment tools to re-evaluate the client. ABS 300 Week One Assessment Scenario Donna, age 14, had consistently been a B+/A- student throughout elementary school and the beginning of middle school. However, in the 8th grade, she started demonstrating difficulty understanding some of her work. Increased difficulties were noted when she was required to work with abstract concepts rather than rely on rote memorization. Donna had always been fascinated with flowers, and she could remember the details of hundreds of different species of wild and domestic flower she encountered. Donna’s classmates and cousins thought she was odd, and her mother said that Donna was frequently picked on—at times without even realizing she was being made fun of. Donna was described as a confused and socially awkward girl who tended to keep to herself. The incident that led to her first psychological evaluation occurred after one of her classmates teased her repeatedly over several days to the point of making Donna upset. Donna decided to write a threatening note to the student as a warning for him to stop. The note included details of which species of flowers would be found growing on top of the place he would be buried. The boy’s parents brought the note to the principal and Donna was suspended from school and charged with terroristic threatening. The school ordered a psychological evaluation and risk assessment before they allowed her to return to school. Donna was observed to have awkward mannerisms, and she smiled at what appeared to be inappropriate times, for example, when she was talking about the teasing at school. She made very poor eye contact in ways that were atypical for her culture, and she had a difficult time staying on topic, frequently shifting the topic of conversation onto her interest in flower. Donna’s intelligence was found to be in the upper limits of the average range on the Wechsler Intelligence Scale for Children, Fifth Edition (WISC-V). The Gilliam Asperger’s Disorder Scale as rated by Donna and her mother together was in the clinically significant range, with her largest deficits being reflected in her social interactions scale. There were also deficits noted in pragmatic skills, restricted patterns of behavior, and cognitive patterns. Problems were also noted with reciprocal social interaction skills, communication skills, and stereotyped behaviors, interests, and activities. Donna’s QEEG results showed multiple abnormalities. Her right parietal-temporal lobe showed excessively slow activity. This is an area important for facial recognition and empathy. She also had excessive mid-line frontal hi-beta, something that is often seen in those with mental rigidity and obsessive thinking. Multiple problems in coherence were noted, reflecting cognitive inefficiency in her mental processing. Excessive connectivity was noted in the frontal lobes areas and there were excessive disconnections between her frontal lobes and the central and back parts of her brain. Donna was diagnosed with Asperger’s Disorder in accordance with the DSM-IV criteria and referred for academic and social skill instruction. Approximately, two years later, Donna, age 16, and her parents met with the school’s child study team to review a revised Individualized Education Program (IEP). Although Donna’s academic achievement was approximately one grade level behind her current 11th grade placement, she had made significant academic and social gains over the past two years. Ms. Kraut, Donna’s mother, noted a change in her daughter’s diagnosis from Asperger’s Disorder to Autism Spectrum Disorder, Level 1—Requiring support, Without accompanying intellectual impairment, Without accompanying language impairment. The school psychologist explained that the change in diagnoses was due to changes in the manual used to assign psychiatric diagnoses (the Diagnostic and Statistical Manual for Mental Disorders, Fifth Edition, DSM-5). Ms. Kraut insisted that her daughter was not autistic and demanded a re-evaluation. The psychologist stated that there was no data supporting a re-evaluation at this time. Ms. Kraut left the meeting, declaring that she would pay for a second opinion herself. When Mr. Kraut met with the private psychologist she stated that her daughter needed “a more appropriate diagnosis.” The private psychologist reviewed the previous records and explained the change to the DSM-5 and the professional directive that individuals with a well-established DSM-IV diagnosis of Asperger’s disorder should be given the diagnosis of autism spectrum disorder. Ms. Kraut responded, “I do not care! My daughter is not autistic. She can be ADHD or LD, but she cannot be autistic. Do you understand me?!”

The ability to stay focused and prioritize the med pass as distractions continue to occur in the middle of it is critical to safety and also has its set of challenges through all areas of nursing.  The demand for the nurses attention is usually always more than they have to give and this can be compounded by staff that do not possess critical thinking skills and ask questions before processing through information first.  The nurse is responsible to multi task while using EBP and critical thinking skills in a flurry of never ending activity.  I enjoy the end result when all care and tasks are complete for your assignment and the feeling of accomplishment throughout the med pass or other duties that relate to your daily tasks.  I also enjoy the ability to mentor other staff members that may have been the ones asking multiple qustions that were in the realm of their capacity to problem solve.  The amount of work that a nurse can complete when demands continue to barrage you are incomprehensible at times.  These are days that I am proud to be a nurse and possess critical thinking skills.
