Have you identified the past diagnosis (or diagnoses if more than one exists in the file) and explained how the diagnosis was made? Specifically what tests were done?

Your final project should discuss the following critical elements. Make sure you review your final project before you submit it and make sure it discusses all the critical elements and answers the questions.
Patient History Analysis
a) Did you summarize the patient history explaining key patient demographics and family history that could be risk factors for common diseases?
b) Have you identified the past diagnosis (or diagnoses if more than one exists in the file) and explained how the diagnosis was made? Specifically what tests were done?
c) Did you discuss the symptoms the patient showed according to the file? Did you discuss why and how these symptoms led the doctors to order certain tests?
d) Did you include what alternate diagnosis (or diagnoses) these symptoms could have indicated? Did you use evidence-based resources to support your conclusions?
e) Using supportive details from peer-reviewed resources did you explain the pathophysiology of the diagnosis? In other words how does the diagnosed disease develop and progress in the body?
f) Have you identified the past prescribed medications the patient is taking and explained the purposes of their larger pharmacological groupings?
g) Did you explain what symptoms the specific medications are meant to treat using resources to support your claims about the impact of the medication on the symptoms?
h) Did you illustrate how these medications impact the body and its functions? Did you include examples to support your explanations?Recent Visit Analysis
a) Did you explain why the patient has returned to the doctors office? Did you discuss what symptoms and signs the patient is experiencing?
b) Did you analyze the new symptoms and signs to determine whether the past diagnosis is still a reasonable conclusion or could have been a misdiagnosis? Did you use specific information from both the recent visit and the patient history to inform your analysis?
c) Based on the new signs symptoms and potential diagnosis (if the doctor has made a new diagnosis) did you discuss what new or potential treatments would be appropriate? Why?
III. Identification of Record Inaccuracies
a) The patient file contains several inaccuracies and inconsistencies. Using your knowledge of medical terminology anatomy and physiology did you articulate three issues you have identified?
b) Did you explain in detail what makes the identified issues a problem in terms of patient health and record keeping? Did you use appropriate medical terminology references to anatomy and concepts of normal physiology where appropriate?
c) Did you discuss the impact of the issues on the patient the coding system and the billing system if they had not been caught using the appropriate terminology?
d) Did you illustrate how you would work to address each issue with specific detail regarding who you would pull into the discussion and who would be responsible for the particular details of each issue in a real medical setting?

Four evidence-based interventions on the treatment of hypertension in adults were reviewed. The first article titled Home Blood Pressure Tele monitoring: Rationale for Use Required Elements and Barriers to Implementation in Canada by Wood Boulanger & Padwal (2017) asserts that the use of home blood pressure monitoring for hypertension diagnosis and management is highly recommended. To help patients adhere to the recommended home protocol they recommended the use of tele-monitoring because its use combined with protocol zed case management leads to statistically significant and clinically important blood pressure reductions (Wood Boulanger& Padwal 2017).
Roesler Binotto Iochpe Palomba & Tizatto (2015) also noted that chronic diseases including hypertension and diabetes should be addressed given an increase in healthcare risk factors such as population aging. They conducted a study to Improve Preventive Healthcare with a User-centric Mobile Tele-monitoring Model and found that by following a steady monitoring strategy over an elderly population and then numerous deaths could be avoided. The article suggests a mobile health promotion designed to monitor remotely a patient’s vital signs in real time (Roesler et al. 2015)
Chen et al. (2013) also evaluated the effectiveness of a home telehealth service in hypertension control. The study used two groups based on the frequency that they measured blood pressure i.e. one group measured its BP more than 3 times a week while the other made less than or equal to 20 measurements a month. It is important to note that some patients were provided with the Citizen Tele-medical Care Service System (CTCS) home monitoring equipment while others used regular BP measurement. According to their results patients who used CTCS tele-monitoring achieved better BP control than those who self-measured BP at home only.
Finally Franssen et al. (2017) asserted that tele-monitoring in the management of hypertension is efficient. They however note that its advantages over self-monitoring are not clear. The study aimed at evaluating whether using self-monitoring results in lower BP compared to tele-monitoring. After randomizing participants to usual care self-monitoring alone or self-monitoring with tele-monitoring for 12 months the study revealed that those who used self-monitoring with tele-monitoring had better control of their BP Franssen et al. (2017).

Briefly describe how alcohol dehydrogenase participates in EG toxicity, include the names of the toxic products that are formed and, generally, how the accumulation of these toxic products can lead to life-threateningsituations.

if you refer to any of these journal articles in answering any of the Skills test questions you probably should use an appropriate referencing system. This will include both in-text referencing and a reference section at the end of the work in question.
A common (and recommended) referencing style to use is the Harvard format – this is the format used in the examples below.
For in-text referencing include the author(s) surname followed by a comma and the year of publication in the following formats:
(Jones, 2011) – if a single author(Little and Large, 2012) – if 2 authors(Legion et al., 2012) – if 3 or more authors – watch the punctuation!!
For a reference section at the end of a work list the articles in alphabetical order according to first author surname. You will also need to include details of author initials, year of publication, title of paper, journal title, volume number and page numbers.
Example (again pay attention to punctuation):
Aardvark, Q. A., Echidna, B. F. and Pangolin, Z. U. (2012) The nutritional benefits to eating ants. Journal of Insectivores 333, 21-45.
Abbott, A.J., Rudd, K.M. and Gillard, J. E. (2013) The Australian political landscape: apocalypse now. Political Irrelevancies 90, 623-685.

Ethylene glycol (EG) is a well-characterised metabolic poison that causes considerable harm to humans and animals and can lead to fatalities. EG interferes with mammalian alcoholmetabolism.
Briefly describe how alcohol dehydrogenase participates in EG toxicity, include the names of the toxic products that are formed and, generally, how the accumulation of these toxic products can lead to life-threateningsituations.Including a diagram/figure could behelpful.(4marks)
Imagine you are medical professional working in the Accident & Emergency department in a major hospital. Briefly describe the treatment options and how you would treat a patient that presents with EGpoisoning?(4marks)
EG acts as an enzyme inhibitor; that is a molecule that binds to an enzyme (in this case ADH) and affects its normal mechanism of action and its normal activity. Which type of reversible enzyme inhibition is utilized in the effective treatment of EGpoisoning?(1marks)

Describe the participant sampling, treatment protocols, assessment strategies and potential difficulties in researching the benefits of social support and existing mental health resources for those diagnosed with “schizophrenia” in a small rural communityin

You are asked by State Mental Health authorities to help understand how to best support those who are seriously mentally ill in rural communities. In particular they want to know how to provide “social support” for those with a diagnosis of “schizophrenia” because some evidence indicates that it reduces relapse rates. Describe the participant sampling, treatment protocols, assessment strategies and potential difficulties in researching the benefits of social support and existing mental health resources for those diagnosed with “schizophrenia” in a small rural community in western NSW (let’s call it Wobegone, population 608; baserate of those with the diagnosis of interest is 9%). The nearest Base Hospital is 250 km away in Dubbo. There is one resident, registered generalist psychologist in Wobegon, 2 community mental health nurses who visit Wobegon 3 days per month from Dubbo, and a psychiatrist who visits once per month from Sydney to monitor medication.
