Write a 3-4 page risk management policy and procedure for a health care organization. Analyze a specific issue that occurred in a health care organization and apply risk management best practices to it for the purpose of early risk identification and risk reduction or elimination in the future.
Health care organizations have always searched for ways to identify and reduce risks. An organization’s ability to identify and analyze its risk exposure is a determining factor in the effectiveness of its risk management program (Hoarle, 2015). Early identification and analysis are essential. 
Current health care risk management practices developed in the mid-1970s as a result of a surge in malpractice suits. These suits caused rapid increases in claims costs for the industry and later in insurance premiums. Today, health care delivery systems and organizations realize the value of risk management and have developed formalized programs (Hoarle, 2015). In addition, organizations have established mechanisms to review potential incidents of risk and safety concerns (Pelletier & Beaudin, 2018). While risk management programs are responsible for daily management and risk operations, all health care stakeholders are responsible to participate in activities that will reduce unnecessary risks and improve safety and quality (Hoarle, 2015). 
This second course assessment consists of two parts. You are to assume the role of a new risk manager within your organization’s risk management department. According to your director, employees lack awareness of the organization’s risk management program. Likewise, departments inconsistently apply risk management principles. As a result of these deficiencies, your director has given you your first assignment.  Part One: Risk Management Policy and Procedure
Your director has asked you to write a formal risk management policy and procedure for the organization.  Part Two: Application of Risk Management Principles to a Specific Incident

As the new risk manager in your health care organization, your director has assigned you responsibility for drafting the organization’s risk management policy and procedure. This assignment stemmed from your director’s perception that employees lacked knowledge and awareness of risk management’s contribution to furthering the organization’s safety and quality improvement efforts. Likewise, your director also saw evidence that departments within the organization were inconsistently applying risk management principles to their daily work practices.

Risk Description: Which potential risk to your organization from the Vila Health: Patient Safety simulation are you analyzing? These included patient identification error, medication error, and a HIPAA/privacy violation. Risk Implications: What are the risks to patients, employees, and to the organization if this particular risk is not addressed? In other words, what could happen if the organization chooses to do nothing?  Risk Identification: What risk management strategies and techniques will the organization employ to identify this type of risk in the future? The integration of all professionals seemed beneficial when relating with others towards a holistic being. Professionals agree in the importance of an understanding of varied professionals only utilizing their specific specialization and donât counsel beyond their boundaries. Clinicians must collaborate with spiritual professionals as pastors, chaplains, medical and cultural based healers to help the whole person with mind, body, and spirit. The study looked at different models of which to work as complimentary on each other with no such competition. The generalist-specialist model consists of the spiritual aspect care of the whole person among the medical professions while implementing the chaplains within the team of healthcare. The existential functioning model offers a more in-depth care such as psychiatry which required increased time and structure. An open pluralist model refers to those who may require more serious challenges, such as differing spiritual and cultural traditions (Balboni, Puchalski & Peteet, 2014).
Varied concepts describing a lifeâs journey, referring to wellderly as an individual who has achieved knowledge and is comfortable with lifeâs challenges with open-minded, curiosity and acceptance. It is a positive aspect consisting of a process of hope and care. On the other side is what is referred to as the illderly, which is the positive concept. Individuals with despair and disgust with self may experience the end of life with feelings of fear and regrets, negative outcomes (Nilsson, Bülow, & Kazemi, 2015).
I agree that for a person to work towards a process of healing it must be done so in a holistic manner. I donât believe there can be a balance at any time unless all aspects of what makes that person whole are worked on together. Even with this, there will always remain the importance of helping the individual to know self well so hopefully, they are prepared when lifeâs challenges come about.

James is a community counseling student who is interning with the College Counseling Center at the local university. James is a 25 year-old Caucasian male, who attended a private high school in a small rural town. In terms of being a counselor, James admits that one of his challenges is that he has not experienced much diversity in his relationships. He states that this is not a significant issue because he plans on opening a private practice in his hometown as soon as he graduates. Since he “knows the town” he doesn’t feel that this is a big deal. Also, in terms of finding clients, James states that he is sure he will be able to get referrals because his father works for the town newspaper and will run an ad as soon as he moves home. James admits he has no clinical experience but is pleased, upon arriving at his internship site, to see that his ex-girlfriend is going to be his assigned clinical supervisor. Since they have not seen each other in years, James figures that meeting once a week will give them a good opportunity to catch up on “old times”. James also figures that she could continue to supervise him in private practice until he receives his license. James is relieved because he knows she won’t mind, since he will be graduating in nine months, if he puts her credential, Licensed Professional Counselor, after his name since she is supervising him. After discussing private practice, James asks his ex-girlfriend if he can begin to see clients right away because he is anxious to get experience and begin working on his practice. Since she figured he would be a pretty good counselor, she agreed and paired him up with Lisa, a 21 year-old Latina female. During the first session, James was running late and was unable to fully cover all of the intake documents with Lisa. He put them aside and said, “We don’t really need to review these. It is just paperwork required by the Center. Please sign here.” As the counseling session continued, James thought he might have recognized symptoms associated with depression including decreased motivation, loss of appetite and overall dissatisfaction. He thought about bringing these up with his supervisor but forgot once he left the session. James continued to meet with Lisa. The first few sessions focused mainly on her recent break-up with her boyfriend and the resulting academic difficulties. She was very concerned that her career dream of becoming a lawyer may be in jeopardy. In the third session, Lisa casually mentioned that she began to experience depressive symptoms as a child when her mother’s partner began to physically abuse her at the age of 5½ years. Lisa told her mother about the abuse, but she did not intervene. Shortly after the physical abuse began, the man began to also sexually abuse her. Once again, Lisa reported this to her mother and also to a teacher at her school, but nothing was ever done. Lisa reported that when she was 7, she placed an unknown prescription medication of her mother’s into this man’s alcoholic drink. Later that same day, the man experienced a stroke and was rushed to the hospital where he later died. Lisa also reported learning that her mother’s partner’s stroke and subsequent death were related to the combination of prescription medication and alcohol. The client has never told anyone other than James that she had placed medication in this man’s drink.
