In the chapter, “How Community-Based Organizations Are Addressing Nursing’s Role in Transforming Health Care,”(SEE ATTACHED DOCUMENT HAVE THE CHAPTER TO READ))))) the authors describe the community as the focus of advocacy action for change. They list a series of questions that help community teams develop organizing principles to guide their interventions, and the aim is to attack social determinants of health to make community change.

Suppose a community team identifies lack of access to exercise modalities as a public health issue. If the team were to determine that pedestrian walkways need to be constructed, discuss at least two factors that would facilitate the development of the walkways and two that would hinder the development. For the hindrances, discuss ways that the team might work to overcome those hindrances.

The RN to BSN program at Grand Canyon University meets the requirements for clinical competencies as defined by the Commission on Collegiate Nursing Education (CCNE) and the American Association of Colleges of Nursing (AACN), using nontraditional experiences for practicing nurses. These experiences come in the form of direct and indirect care experiences in which licensed nursing students engage in learning within the context of their hospital organization, specific care discipline, and local communities.

This assignment consists of both an interview and a PowerPoint (PPT) presentation.

Assessment/Interview

Select a community of interest. It is important that the community selected be one in which a CLC group member currently resides. Students residing in the chosen community should be assigned to perform the physical assessment of the community.

  2. Interview a community health and public health provider regarding that person’s role and experiences within the community.

Description of community and community boundaries: the people and the geographic, geopolitical, financial, educational level, ethnic, and phenomenological features of the community as well as types of social interactions, common goals and interests, barriers, and challenges, including any identified social determinates of health.
  2. Summary of community assessment: (a) funding sources and (b) partnerships.
  3. Summary of interview with community health/public health provider.

Jeanne Blum, RN, is a nurse on a LDRP unit. Recently, the policy and   procedures manual for Jeanne’s unit included the premature rupturing of membranes of a laboring patient    as a practice acceptable for nurses to perform. Jeanne    and some of her coworkers shared their concern over   lunch about this new responsibility.They felt uncomfortable with the possibility of cord prolapse and    other potential medical complications resulting from    this practice. The purpose of this assignment is for you to incorporate research evidence into a clinical model for a practice change. Using the Iowa Model for Evidence Based Practice as a guide, develop a concept map/schematic that shows the pathways necessary for a clinical change that you would like to see implemented in your current practice setting.

The concept map should include the 7 steps below in schematic format:

1. Formulate a clinical problem that is nursing sensitive and patient focused. Develop a PICO statement to articulate a research question>

Transforming healthcare

Transforming healthcare and improving quality are priorities for the profession of nursing.  The following activity will help you explore key national priority health care areas for quality improvement as identified by the Institute of Medicine.  Begin by selecting a key healthcare priority area and a population.
