Transcultural Assessment-Week 3

Question 1

In assessing Veronica, the nurse will need knowledge of bio cultural aspects of chronic diseases such as Down syndrome. Secondly, the nurse needs knowledge about past medical history and family history to identify the presence of genetic disorders. The information include the patient’s culture and primary healthcare methods. The nurse should also include whether the family has sought other types of healing including from magico-religious solutions. The nurse should have adequate knowledge of Down syndrome and its prevalence among the cultural group.
The genetics of Down syndrome are important when dealing with Veronica’s parents. Understanding the genetics of the condition will help the nurse explain to the parents about the cause. It helps eliminate possibly held beliefs that maybe associated with the symptoms such as delayed growth. A nurse who understands how the condition occurs will also advice the parents further in regard to getting children in the future.
The primary concern I would have with the parents is misunderstanding about the cause of the challenge. Some cultures relate diseases with supernatural factors such as gods and curses (Andres & Bolye, 2016). Due to the rarity of the symptoms, the parents may associate them child with such factors. As such, the nurse may have hard time explaining the condition to the parents.
Question 2

Sickle cell is a condition that is prevalent among blacks. However, being black is not a guarantee that one will develop the condition. As such, to help Michelle understand the disease and its genetics, the nurse should start by explaining the chances that she may develop the disease. This include asking about family history of the disease to identify whether there are other members of her family with the condition.
It is reasonable to tell Michelle that the baby can be tested for Sickle cell after birth. Early diagnosis of sickle cell is key to management as the disease makes a child vulnerable to diseases (Vila Cuenca et al., 2020). By testing the child, one can get proper treatment and protect them from health challenges. Moreover, telling Michelle that the child can be tested prepares her psychologically and cognitively for an outcome where the child is positive for sickle cell disease.
In the discussion with Michelle, the nurse should discuss culturally appropriate measures for management of sickle cell anemia. According to Vila Cuenca et al. (2020), stem cell transplant may cure sickle cell. The nurse should ensure that they inform Michelle of the possible treatment and management modalities for sickle cell. Other information to include are how to relieve pain and prevent complications.

There are chances that a person of Mediterranean descent will have low hemoglobin levels in their blood due to the high prevalence of thalassemia in this area. Thalassemia or Mediterranean anemia is a hereditary disease that is common among people of Mediterranean origin (Qingmei et al., 2016). During assessment, the knowledge about the prevalence of thalassemia help nurses to tailor their tests towards the disease. Therefore, the nurse may look for signs and symptoms of anemia in the patient from the Mediterranean descent.
Low hemoglobin level is related to poor health including which include anemia with such symptoms as body weakness, dizziness, shortness of breath, headache, cold and pale skin. These signs and symptoms are related with a decline in the oxygen carrying capacity of blood (Cavero-Redondo et al., 2017). The individual with low hemoglobin levels may at times experience breathing difficulties and other challenges due to the inability to carry oxygen.

The Recruiting Process

Discussion: The Recruiting Process

How many times have you, or has someone you know, perused jobs on Monster.com, LinkedIn or a clinical listserv? When browsing employment opportunities, what factors motivate you to apply for one position over another?

As a nurse manager, it is critical that you consider the answers to questions such as these when designing your recruitment process. Identifying the approaches applicants use to find jobs can help you broaden your pool of applicants. In addition, knowing how to create accurate job descriptions to advertise for open positions can increase the chances of finding an applicant that is qualified and exhibits skills that you and your workplace desire.

In this Discussion, you examine the process in which a health care organization engages when attempting to recruit qualified and desirable job candidates.

To prepare

Review Chapters 4 and 5 from the course text, Human Resource Management: Functions, Applications, Skill Development. What best practices can be used to identify, analyze, and describe positions within a health care setting? In addition, what considerations and factors should be taken into account when recruiting job candidates?

Reflect upon the recruitment process used in your current organization or one with which you are familiar. Then, select one setting to further investigate.

Consider questions such as the ones below to identify the recruitment methods and steps involved in searching for and attracting potential employees.

What is expected of the nurse manager in terms of recruitment?

What is expected of HR in terms of recruitment?

How are job openings posted and/or advertised?

What challenges or constraints have been experienced with regard to recruiting desirable and qualified applicants?

Submit a brief description of your selected organization’s recruitment process. Include an explanation of who is responsible for each step, how and where positions are advertised, and any unique methods used to recruit employees. Explain the effectiveness of these recruitment processes by citing authentic examples from the setting you selected and this week’s Learning Resources as appropriate.

Read a selection of your colleagues’ responses.

Respond to at least two of your colleagues on two different days using one or more of the following approaches:

Offer and support an alternative perspective on the effectiveness of a colleague’s recruitment process by citing examples from the Learning Resources or from your own research in the Walden Library.

Expand on a colleague’s posting by providing a legal perspective for why a recruitment process should be revamped or changed, based on readings and evidence.

Share an insight on similar advertising and recruitment methods your setting has used, synthesizing information to provide new perspectives.

Financing of Health Care

Discussion: Financing of Health Care

With coinciding concerns about health care costs and the imperative to improve quality of care, health care providers and others face difficult decisions in the effort to achieve an appropriate balance. Such decisions often are addressed in the policy arena. How do policymakers evaluate which health care services should be financed through government programs? How do ethics-related questions and other considerations play into this evaluation process? Is it possible to contain costs and provide accessible, high-quality care to all, or is the tension between cost and care inherent in the U.S. health care delivery system? These questions are central to health care financing decisions in the United States.

For this Discussion, you will focus on the policy decision-making process that determines what types of care are covered by public and private insurers and the ethical aspects of such financial decisions.

To prepare:

Read the case study “Economic Impact of States Declining Medicaid Expansion”  page 190 of the Milstead text( BOOK :HEALTH POLICY AND POLITIC ATTACHED BELLOW) .

Review the information in the Washington Post article “Review of Prostate Cancer Drugs Provenge Renews Medical Cost-Benefit Debate” in the Learning Resources. ( DOCUMENT ATTACHED BELLOW)

Consider how policy decisions currently are made about what will and will not be paid for and what changes, if any, could improve the process.

Reflect on how the Washington Post example illustrates the tension between cost and care.

Post your analysis and assessment of the ethical and economic challenges related to policy decisions such as those presented in the Washington Post article.

How does this type of situation contribute to the tension between cost and care? Substantiate your response with at least two outside resources.

Using Variance Analysis in Decision Making

Assignment: Using Variance Analysis in Decision Making

Having a budget is critical for the financial stability of an organization. Keeping track of how well the organization is actually adhering to the budget, and subsequent identification of why the budget numbers are being missed is equally important. Without this critical “why” piece, it is difficult to make the necessary adjustments to the budget or to organizational behavior that might be promoting overspending.

A good budget is built with thoughtful consideration of future costs and revenue. Though your budget is formulated with expected figures in mind, the actual resulting values may vary considerably. This variance–from projected to actual–can be a pleasant surprise or a fiscal nightmare and can make financial decision making difficult. Fortunately, variance analysis can enable management to determine why variance occurred and what can be done to mitigate its effects.

For those not comfortable with the use of Microsoft Excel, this week’s Optional Resources suggest several tutorials.

To prepare:

  • Review the information in this week’s Learning Resources dealing with variance analysis, how it is calculated, and how it can be used in decision making.
  • View the video  Week 8 Application Assignment Tutorial: Variance Analysis, provided in this week’s Learning Resources.
  • Use the Week 8 Application Assignment Template, provided in this week’s Learning Resources, to complete this assignment.   Carefully examine the information in each of the scenarios and provide the necessary calculations.  Using this information will help you answer the questions.

Nursing Role and Scope

Discussion Question Week 7

Nursing by profession is very wide. Nursing career paths which are supported by graduate-level academic programs are expert clinician, clinical nurse leader, nurse educator, nurse executive, and nurse researcher. As a nurse, one can view it as a career or as a job. Nursing as a job entails obtaining the least amount of education required to obtain a license for a nurse. This is done through obtaining the minimum continuing education units which are required for licensure (Shea, 2016). Through this, one goes on with the job as long as they are located some amount of work which results in good pay. One just studies for the license to obtain the job, and get paid.

Nursing as a career entails furthering nursing studies to obtain a bachelor of science in nursing and an advanced nursing degree. To view nursing as a career entails one doing further researches which help in nursing (McGee et al., 2019). Also, one can enroll in leadership positions to guide others in the field of nursing. In my view, I consider nursing as a career. When doing a task, it is always good and necessary to do it out of passion and more efforts. I view nursing as a jovial career which entails many aspects. Interacting with patients and helping them the best from passion, knowledge and experience. It does not entail just performing tasks because one is employed to do so. It is not more of pay, but more of further studies and passion.

Professional goals related to nursing are; healthcare should be delivered appropriately, practitioners should be given enough training to help in the career, there should be enough nursing educational preparation before one qualifies to be a nurse, nurses should utilize their education and training in the right manner, nurses should undergo through an improved education system which promotes academic progression, there should be effective workforce planning and policymaking which require improved information structure and better data collection (Masters, 2017).

Many students who enroll in the field of nursing do it out of passion and desire. Patient care being the primary core objective in nursing, many strive to ensure that they offer the best they acquire from the training. An interview is one of the ways of showcasing one’s professional self. In an interview for my ideal future nursing role, I would take the following steps; know myself- the first is the thing is to acknowledge that am a nurse by profession and this is an interview of a duty of call (Shea, 2016).

Therefore, I have to follow the right procedures in the interview. Know the healthcare organization I will be interviewed from-I have to acknowledge the healthcare center where I will be interviewed from and what is required. I should also make up a follow up of the characteristics of the organization. Prepare my documents ahead of time before the interview- I should ensure that I have all the necessary documents required for the interview to avoid collisions during the interview.

Look professional- I should ensure I look presentable as a nurse during the interview. One’s appearance matters the most, and it is good to have a good appearance. Plan for the day- I should make a good plan which will suit my interview appropriately (Shea, 2016). Get some sleep- I should have enough sleep and rest before the interview to avoid confusions and forgotten during the interview.
