On Monday morning, her mother took her to her doctor. Upon completing the physical exam, he told Melissa the lymph nodes were enlarged in her neck and she had a fever. Ahmed has worked as a phlebotomist in the local hospital for the last 7 years. Last year, he began to complain of watery, nasal congestion and wheezing whenever he went to work. He suspected he was allergic to something at the hospital because his symptoms abated when he was at home over the weekends. One day he arrived at work for the morning shift and put on his gloves. Within minutes, he went into severe respiratory distress requiring treatment in the emergency ward. It was determined at that time his allergic response was due to latex exposure.

Ahmed experienced a type I, IgE-mediated hypersensitivity response. How can this be determined by his signs and symptoms? How might another type of latex hypersensitivity reaction present?

Inflammation, Tissue Repair, and Wound Healing

Carlton, a six-year-old boy, was playing on a sandy beach with his mother. He began to run along the shoreline when he stepped on the sharp edge of a shell, giving himself a deep cut on his foot. His mother washed his foot in the lake and put on his running shoe to take him home. One day later, Carlton’s foot looked worse. The gash was red and painful. The foot was warm to touch and appeared swollen. Carlton’s mom put some gauze over the wound and prepared to take him to the local community health clinic.

Acquired Immunodeficiency Syndrome

Patience is 29 years old and has been HIV positive for nine years. She has remained asymptomatic and is not taking antiretroviral medication. Recently she was at the drop-in clinic to talk to a public health nurse about having a baby through artificial insemination. She said she had met a man who wanted to marry her and have children with her, but she was concerned about the baby contracting HIV. Her latest blood tests indicated her CD4+ count was 380/µL. The PCR test indicated her viral load was 850. The nurse referred her to the physician to discuss antiretroviral therapy during her pregnancy.

What are the factors that increase the chance of HIV transmission from mother to infant, and how the transmission occurs?
Patience was told that after she became pregnant, she would begin HAART therapy. Describe what this therapy is and what particular antiretroviral medication would be particularly useful to her during her pregnancy. Case Study 6

Blood Cells and the Hematopoietic System

Charlie is a 53-year-old man with non-Hodgkin lymphoma. His treatment has been only modestly successful in delaying the progression of the disease, and he has recently relapsed. His medical team decided to administer aggressive chemotherapy. Knowing that the intensive treatment would have a destructive effect on Charlie’s bone marrow, they removed stem cells from his blood before the chemotherapy began. Afterward, the stem cells were returned by IV to reestablish his bone marrow function.

Disorders of White Blood Cells and Lymphoid Tissues

Max is a 60-year-old living in Iowa. For the 27 years, he has been working in the agricultural industry, particularly in the management of corn production. Recently he began to feel weak during work and tired easily. During the night he woke up sweating, and he often felt unusually warm during the day. Max was also surprised that, in spite of eating regularly, his weight was declining and his work pants were now too large for him. Upon physical examination, his physician noted his inguinal lymph nodes were swollen although Max said they were not sore. Subsequent laboratory tests confirmed follicular, non-Hodgkin lymphoma. Based on the readings assigned and suggested for this week and a review of the seven-stage, sequential framework to promote greater use of EBPH, do the following:

The application of evidence-based principles to a public health issue demands a scientific quantification of said public health issue. An integral component of the community assessment should include the targeted health condition or risk factor being considered, the population affected, the size and scope of the problem, prevention opportunities, and potential stakeholders. To aid in this task, epidemiology and the US surveillance system are applied to track and obtain necessary information about the frequency of the health condition or risk factor in an affected population.

Although data from surveillance systems can be used to obtain baseline and follow-up measurements for target populations, there may be limitations when using the data to evaluate intervention effectiveness for narrowly defined populations. In such a case, it may be necessary to estimate the frequency of disease or other health condition for the target population by using special surveys or appropriate study designs. Understanding the tradeoffs of various study designs will improve how you evaluate the effects of various public health programs and policies.

Review the study designs from the readings assigned and suggested for this week and present a brief summary of the appropriateness and advantages of using each study design as part of your effort to quantify the public health issue that you identified in Week 1 as part of your community assessment.

background of the problem(2 slides):  Over the years, hospital acquired pressure ulcers (HAPU) have been a major concern in hospitalized patients.“More than 2.5 million patients in the United States suffer from pressure ulcers in acute care facilities annually; sixty thousand of those patients die related to the complications of pressure ulcers” (Paul, Hanson, Hasenau & Dunn, 2013, p. 32).HAPU’s are more prevalent amongst patients with restricted/impaired mobility.Because patients are often very ill once admitted to an acute care setting, skin assessments and prevention measures are often delayed after admission.It is stated that “pressure ulcers contribute to extended length of hospital stay, infections, amputations, increased need for specialty consults, increased nursing time, and to the potential for hospital readmission”

Analysis of Problem (3-4 slides):  According to the case study, HAPU rates on Unit B have risen significantly over the past 6 months.There is one Wound and Skin Care Nurse (WSCN), servicing the entire facility Monday-Friday from 8a-5p.The WSCN visits patients based on a computer generated list of patients at risk for skin breakdown.A problem arises when patients that need to be seen do not populate onto the daily list.The WSCN provides all the treatments due to the fact that direct care nurses state that they are not familiar with how to care for the patient with skin breakdown unless the WSCN provides specific directions.Missed OpportunitiesFloor nurses don’t know the best available evidence for prevention of skin breakdown.Patients are only assessed once during the midnight shift.In order for patients to be seen by the WSCN they must appear on a computer-generated list.

As a nurse engaged in evidence-based practice, it is important to recognize how statistics and other data analysis tools are used to generate and assess evidence. Most nurses need only a foundational understanding of statistical tools and terminology to understand the majority of research studies. As a nurse, you should be able to recognize the most commonly used statistical tests, how and when they are used, and how significance is determined.

 

In this Discussion, you examine different types of statistics and statistical tests, when and why these particular tests would be selected for use, and, most importantly, what the results indicate. To this end, you will be assigned to a group by Day 1 of this week.
