Practicum Journal Entry: Analyzing an Ethical Decision

Assignment 1: Practicum Journal Entry: Analyzing an Ethical Decision

In your role as a PMHNP, you will encounter several situations that will require your ability to make sound judgments and practice decisions for the safety and well-being of individuals, families, and communities. There may not be a clear-cut answer of how to address the issue, but your ethical decision making must be based on evidenced-based practice and what is good, right, and beneficial for patients. You will encounter patients who do not hold your values, but you must remain professional and unbiased in the care you provide to all patients regardless of their socio-demographic and ethnic/racial background. You must be prepared to critically analyze ethical situations and develop an appropriate plan of action. For this Assignment, you review the literature and discover the various ethical dilemmas PMHNPs encounter and how these issues are typically addressed in your state.

Learning Objectives

Students will:

  • Analyze      salient ethical issues in  child psychiatric      mental health practice
  • Compare      ethical dilemmas with state health laws and regulations
  • Analyze      ethical decision-making processes

To prepare:

  • Review      the literature for moral/ethical issues encountered by a PMHNP
  • Select      one of the articles you found that      was published within the last 5      years to use as a focus for this assignment.

Write a 2-page paper in which you do the following:

  • Summarize the moral/ethical      issue in the article (no more than 1 paragraph).
  • Describe the moral and      ethical dilemmas surrounding the issue.
  • Analyze the ethical issue      and compare them to the state health laws and regulations in your state. Please use( Texas) law
  • Outline the process of      ethical decision making you would      use to address this ethical dilemma.

EBP practitioners

As your EBP skills grow, you may be called upon to share your expertise with others. While EBP practice is often conducted with unique outcomes in mind, EBP practitioners who share their results can both add to the general body of knowledge and serve as an advocate for the application of EBP.

In this Discussion, you will explore strategies for disseminating EBP within your organization, community, or industry.

To Prepare:

Review the Resources and reflect on the various strategies presented throughout the course that may be helpful in disseminating effective and widely cited EBP.
This may include: unit-level or organizational-level presentations, poster presentations, and podium presentations at organizational, local, regional, state, and national levels, as well as publication in peer-reviewed journals.
Reflect on which type of dissemination strategy you might use to communicate EBP.
Post at least two dissemination strategies you would be most inclined to use and explain why. Explain which dissemination strategies you would be least inclined to use and explain why. Identify at least two barriers you might encounter when using the dissemination strategies you are most inclined to use. Be specific and provide examples. Community, Culture, and Nursing

Week 1 Discussion

Previous  Next

Community, Culture, and Nursing

The discussion assignment provides a forum for discussing relevant topics for this week based on the course competencies covered.

By the due date assigned, post a response to one of the two discussion topics below, using information from the lectures, reading assignments, library resources, and Internet resources. All responses should be posted to the appropriate topic in the Discussion Area.

Topic 1: Community-Based Nursing

Nursing services play an important role in caring for patients and families in the community and providing them the support they need. For patients with problems ranging from chronic health conditions to need of elder care, nursing has proved itself an important part of the healthcare team. In fact, many people now prefer sending their elderly parents to a daycare for nursing, rather than to a long-term care facility.

For this topic, interview a community-based nurse or nurse practitioner who sees individuals in the home setting. Ask the following questions:

  • What are your job responsibilities?
  • Does a correlation exist between your job description and your practice?
  • Are you involved in policy formulation that affects client services?
  • Do you belong to a professional organization related to your current job position?

Topic 2: Culturally Competent Care

A broad range of cultures exists in the world today. Nursing professionals often interact with people from cultural backgrounds that differ from their own.

  • Share an experience where you cared for a patient in the healthcare setting who was from a culture different from your own. How did you accommodate this patient’s needs? How would you apply the Purnell Model to future cultural encounters?
  • Which one of the twelve domains of culture from the Purnell Model do you feel is most important? Why?

Citations should conform to APA guidelines. You may use this APA Citation Helper as a convenient reference for properly citing resources or connect to the APA Style website through the APA icon below.

APA

Before the end of the week, comment on at least two of your peers’ responses. You can ask questions or respond generally to the overall experience. Be objective, clear, and concise. Always use constructive language, even in criticism, to work toward the goal of positive progress. All comments should be posted to the appropriate topic in the Discussion Area.

nonparametric tests.

To prepare:

Review the articles presented in this week’s Learning Resources and analyze each study’s use of nonparametric tests.

Critically analyze each article, considering the following questions in your analysis:

What are the goals and purpose of the research study each article describes?

How are nonparametric tests used in each study? What are the results of their use?

Why are parametric methods (t tests and ANOVA) inappropriate for the statistical analysis of each study’s data?

What are the strengths and weaknesses of each study (e.g., study design, sampling, and measurement)?

How could the findings and recommendations of each study contribute to evidence-based practice in the health care field?

Reflect on the quantitative statistical analyses presented throughout this course in the research literature, the Learning Resources, media presentations, and those articles you reviewed for your abbreviated research proposal.

Ask yourself: Which method is most commonly used in research studies that pertain to my area of nursing practice, and why this might be so?

TO COMPLTE

Post 1-2 PAGES PAPER ON : A cohesive response addresses the following:

Critically analyze each article, including the items noted above.

Identify one statistical analysis method that you found recurring in many of the articles you used in your literature review for your research proposal. This method does not necessarily have to be nonparametric.

Based on your area of nursing practice ( family Nurse Practitioner) , which method of statistical analysis is most frequently used in the research literature? Why do you think other forms of statistical analysis are less frequently used? Provide a rationale for your response.

 Social Policy

As emphasized in this week’s media presentation, all nurses need to be familiar with the laws and regulations that govern their practice: their state’s Nurse Practice Act, ANA’s Nursing: Scope and Standards of Practice, specialty group standards of practice, etc. In addition, basic ethical principles guide nurses’ decision-making process every day. ANA’s Code of Ethics and ANA’s Social Policy Statement are two important documents that outline nurses’ ethical responsibilities to their patients, themselves, and their profession. This said, there is a dilemma: The laws are not always compatible with the ethical positions nurses sometimes take. This week’s Discussion focuses on such a dilemma.

To prepare:

  • Review this week’s Learning Resources, focusing on the information in the media presentation about the relationship between the law and ethics.
  • Consider the ethical responsibility of nurses in ensuring patient autonomy, beneficence, non-malfeasance, and justice.
  • Read the following scenario:

Lena is a community health care nurse who works exclusively with HIV-positive and AIDS patients. As a part of her job, she evaluates new cases and reviews confidential information about these patients. In the course of one of these reviews, Lena learns that her sister’s boyfriend has tested HIV positive. Lena would like to protect her sister from harm and begins to consider how her sister can find out about her boyfriend’s health status.

  • Consult at least two resources to help you establish Lena’s legal and ethical position. These resources might include your state’s Nurse Practice Act, the ANA’s Code of Ethics, ANA’s Nursing: Scope and Standards of Practice, and internal or external standards of care.
  • Consider what action you would take if you were Lena and why.
  • Determine whether the law and the ANA’s standards support or conflict with that action.

Post a description of the actions you would take in this situation, and why. Justify these actions by referencing appropriate laws, ethical standards, and professional guidelines.

Designing and Measuring Outcomes

Designing and Measuring Outcomes

As an advance practice nurse, your work is informed by many factors beyond your direct reach, and your influence extends beyond the immediate context in which you labor. Likewise, the changes you propose as a nurse leader are influenced by multiple systems and, in turn, can have an impact on many different groups. One of the ways to distinguish yourself as a leader is to be knowledgeable about these spheres of influence and to promote awareness of the potential effects that may result from a change.

Indeed, with the fluid and persistently evolving nature of change, it is essential that you stay abreast of latest developments that may affect the practice environment. This is especially critical for maintaining or adapting a change once it has been implemented. As discussed in Weeks 8 and 9, you may need to make adjustments based on evaluation of outcomes; in addition, you must also be prepared for adjustments based on developments within or outside your targeted environment.

To prepare:

  • What are some possible adjustments that you might need to make based on the evaluation of outcomes? Consider insights from the Discussion in Weeks 8 and 9.
  • Consider the evidence-based change you are proposing for your practicum setting: What other systems or components of a system are within the sphere of influence of this change? How could evolving issues internal to the setting, such as budget, staffing shortage, regulations or external to it, such as supply, funding, national priorities, and disasters, affect the implementation—and ultimately the sustainability—of such a change?
  • How might your proposed change, in turn, affect policy or practice at other levels beyond your initial focus?

By Day 3 of Week 10

Post a cohesive response that addresses the following:

  • Describe at least three evolving issues that are internal or external to your practicum setting that could affect the implementation and sustainability of your proposed change. From what systems level do those issues originate (e.g., organizational, local, state, national, global) and what other systems are then involved in response to that factor?
  • How might your proposed change, in turn, lead to shifts at other levels beyond your initial focus?

Effective communication

Effective communication is a staple of our healthcare culture.  Working with patients, peers, and interprofessional teams requires that nurses manage information and evidence toward influencing safe and positive patient outcomes.

Please address the following:

Describe caring attributes of the culture where you currently practice. Which attributes stand out as having significant influence on patients, nurses, and other healthcare professionals?

How do you see effective communication relating to patient outcomes in this setting? What is the evidence for this?
