Therapy for Clients With Personality Disorders

Therapy for Clients With Personality Disorders

Clients with personality disorders often find it difficult to overcome their problems and function in daily life. Even when these clients are aware that they have a dysfunction with their personality and are open to counseling, treatment can be challenging for both the client and the therapist. For this Discussion, as you examine personality disorders, consider therapeutic approaches you might use with clients.

Learning Objectives

Students will:

· Analyze therapeutic approaches to treating clients with personality disorders.

To prepare:

· Review this week’s Learning Resources and reflect on the insights they provide.

· Select one of the personality disorders from the DSM-5. EX Paranoid, schizoid, borderline, narcissistic, avoidant, OCD.

Post a description of the personality disorder you selected. Explain a therapeutic approach (including psychotropic medications if appropriate) you might use to treat a client presenting with this disorder, including how you would share your diagnosis of this disorder to the client in order to avoid damaging the therapeutic relationship. Support your approach with evidence-based literature.

nursing practice

Purpose
Nursing practice is both an art and a science with caring as a central concept to nursing practice and the profession of nursing. Caring is influenced by the nurse’s knowledge, skills, and attitudes formed through life long learning and experiences. The purpose of this assessment is to explore definitions of caring in nursing. Students will create a definition of caring and an artistic expression of their perception on caring.

Criteria for Content

Take a moment to reflect on how you demonstrate caring in your current professional practice.

Create an original artistic representation of your perception on caring in nursing.
Drawings should be formatted on an 8 x 10 document and submitted as a PDF
Poems or short-story – word document 1 page in length
In a one- to two-page written paper, address the following.
The importance of caring to nursing practice and the profession
A personal definition of caring; include principles of holistic care
Definitions of caring found in nursing literature that support your perspective on caring
An explanation of the artistic expression you chose and how it represents caring
Summary of key concepts presented in the paper Preparing the paper
A minimum of three (3) scholarly literature references must be used

Decision Tree

Assignment: Decision Tree

For this Assignment, as you examine the client case study in this week’s Learning Resources, consider how you might assess and treat pediatric clients presenting symptoms of a mental health disorder.

                                                          The Assignment:

Examine Case 2You will be asked to make three decisions concerning the diagnosis and treatment for this client. Be sure to consider co-morbid physical as well as mental factors that might impact the client’s diagnosis and treatment.

At each Decision Point, stop to complete the following:

  • Decision #1: Differential Diagnosis
    • Which Decision did you select?
    • Why did you select this Decision? Support your response with evidence and references to the Learning Resources.
    • What were you hoping to achieve by making this Decision? Support your response with evidence and references to the Learning Resources.
    • Explain any difference between what you expected to achieve with Decision #1 and the results of the Decision. Why were they different?
  • Decision #2: Treatment Plan for Psychotherapy
    • Why did you select this Decision? Support your response with evidence and references to the Learning Resources.
    • What were you hoping to achieve by making this Decision? Support your response with evidence and references to the Learning Resources.
    • Explain any difference between what you expected to achieve with Decision #2 and the results of the Decision. Why were they different?
  • Decision #3: Treatment Plan for Psychopharmacology
    • Why did you select this Decision? Support your response with evidence and references to the Learning Resources.
    • What were you hoping to achieve by making this Decision? Support your response with evidence and references to the Learning Resources.
    • Explain any difference between what you expected to achieve with Decision #3 and the results of the decision. Why were they different?
  • Also include how ethical considerations might impact your treatment plan and communication with clients and their families.

Note: Support your rationale with a minimum of three academic resources. While you may use the course text to support your rationale, it will not count toward the resource requirement.

                                                                                Case #2
Anxiety disorder, OCD, or something else?

8-year-old black male

                                                                    BACKGROUND

Tyrel is an 8-year-old black male who is brought in by his mother for a variety of psychiatric complaints. Shaquana, Tyrel’s mother, reports that Tyrel has been exhibiting a lot of worry and “nervousness” over the past 2 months. She states that she notices that he has been quite “keyed up” and spends a great deal of time worrying about “germs.” She states that he is constantly washing his hands because he feels as though he is going to get sick like he did a few weeks ago, which kept him both out of school and off the playground. He was also not able to see his father for two weekends because of being sick. Shaquana explains that although she and her ex-husband Desmond divorced about 2 years ago, their divorce was amicable and they both endeavor to see that Tyrel is well cared for.

Shaquana reports that Tyrel is irritable at times and has also had some sleep disturbances (which she reports as “trouble staying asleep”). She reports that he has been more and more difficult to get to school as he has become nervous around his classmates. He has missed about 8 days over the course of the last 3 weeks. He has also stopped playing with his best friend from across the street.

His mother reports that she feels “responsible” for his current symptoms. She explains that after he was sick with strep throat a few weeks ago, she encouraged him to be more careful about washing his hands after playing with other children, handling things that did not belong to him, and especially before eating. She continues by saying “maybe if I didn’t make such a big deal about it, he would not be obsessed with germs.”

Per Shaquana, her pregnancy with Tyrel was uncomplicated, and Tyrel has met all developmental milestones on time. He has had an uneventful medical history and is current on all immunizations.

                                                                OBJECTIVE

During your assessment of Tyrel, he seems cautious being around you. He warms a bit as you discuss school, his friends at school, and what he likes to do. He admits that he has been feeling “nervous” lately, but when you question him as to why, he simply shrugs his shoulders.

When you discuss his handwashing with him, he tells you that “handwashing is the best way to keep from getting sick.” When you question him how many times a day he washes his hands, he again shrugs his shoulders. You can see that his bilateral hands are dry. Throughout your assessment, Tyrel reveals that he has been thinking of how dirty his hands are; and no matter how hard he tries to stop thinking about his “dirty” hands, he is unable to do so. He reports that he gets “really nervous” and “scared” that he will get sick, and that the only way to make himself feel better is to wash his hands. He reports that it does work for a while and that he feels “better” after he washes his hands, but then a little while later, he will begin thinking “did I wash my hands well enough? What if I missed an area?” He reports that he can feel himself getting more and more “scared” until he washes his hands again.

                                                         MENTAL STATUS EXAM

Tyrel is alert and oriented to all spheres. Eye contact varies throughout the clinical interview. He reports his mood as “good,” admits to anxiety. Affect consistent to self-reported mood. He denies visual/auditory hallucinations. No overt delusional or paranoid thought processes were apparent. He denies suicidal ideation.

Lab studies obtained from Tyrel’s pediatric nurse practitioner were all within normal parameters. An antistreptolysin O antibody titer was obtained for reasons you are unclear of, and this titer was shown to be above normal parameters.

 Decision Point One

BASED ON THE INFORMATION PROVIDED IN THE SCENARIO ABOVE, WHICH OF THE FOLLOWING DIAGNOSES WOULD THE PMHNP GIVE TO TYREL?

In your write-up of this case, be certain to link specific symptoms presented in the case to DSM–5 criteria to support your diagnosis.

Generalized Anxiety Disorder (GAD)

Obsessive Compulsive Disorder

Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal Infections (“PANDAS”)

ANSWER CHOOSENObsessive Compulsive Disorder

 Decision Point Two

There may also be an element of social phobia developing, but at this point, the PMHNP has not assessed the nature of the school avoidance—that is, why is the child avoiding school. Notice that nothing in the scenario tells us that the PMHNP has assessed this. Zoloft is FDA-approved to treat OCD in children. However, between ages 6 and 12, it should be started at 25 mg orally daily. If starting doses are too high, the child may experience side effects that he associates with the medication and as such, may refuse to take the medication. Starting at too high a dose can result in unfavorable side effects (gastrointestinal side effects are notable in this drug), and we can see that Tyrel is experiencing nausea and decreased appetite. In this case, it is recommended to wait to see if the side effects dissipate. Decreasing the dose to 12.5 mg orally daily for about 3 or 4 days, then going back to 25 mg orally daily may help to overcome the unfavorable side effects. If side effects persist, the PMHNP may need to consider switching to a different medication.Fluvoxamine controlled release is not FDA-approved for use in children with OCD (see “Special Populations: Children and Adolescents” in the Fluvoxamine monograph of Stahl’s Prescriber’s Guide for further details). Fluvoxamine 100 mg orally daily may not be tolerated in the morning secondary to the drug’s sigma-1 antagonist properties, which can cause sedation. Dosing of Fluvoxamine should be such that the larger dose is given in the evening to minimize daytime sedation. It is also worth noting that nothing in the scenario tells us that the Zoloft will not be effective.

ANSWER CHOOSEN: https://mym.cdn.laureate-media.com/2dett4d/Walden/NURS/6660/05/mm/decision_tree/img/pill-blue.pngBegin Fluvoxamine immediate release 25

mg orally at bedtime

·Client returns to clinic in four weeks

·Upon return to the clinic, Tyrel’s mother reported that he has had some

decrease in his symptoms. She states that the frequency of the handwashing

has decreased, and Tyrel seems a bit more “relaxed” overall.

·She also reports that Tyrel has not fully embraced returning to school, but that

his attendance has improved. She reported that over this past weekend, Tyrel

went outside to play with his friend from across the street, which he has not done in a while.

Guidance to Student

In terms of an actual diagnosis, the child’s main symptoms are most consistent with obsessive-compulsive disorder. There may also be an element of social phobia developing, but at this point, the PMHNP has not assessed the nature of the school avoidance—that is, why is the child avoiding school. Notice that nothing in the scenario tells us that the PMHNP has assessed this.

Fluvoxamine immediate release is FDA-approved for the treatment of OCD in children aged 8 years and older. Fluvoxamine’s sigma-1 antagonist properties may cause sedation and as such, it should be dosed in the evening/bedtime.

At this point, it would be appropriate to consider increasing the bedtime dose, especially since the child is responding to the medication and there are no negative side effects.

Atypical antipsychotics are typically not used in the treatment of OCD. There is also nothing to tell us that an atypical antipsychotic would be necessary (e.g., no psychotic symptoms). Additionally, the child seems to be responding to the medication, so there is no rationale as to why an atypical antipsychotic would be added to the current regimen.

Cognitive behavioral therapy is the psychotherapy of choice for treating OCD. The PMHNP should augment medication therapy with CBT. If further assessment determines that Tyrel has social anxiety disorder, CBT is effective in treating this condition as well.

Elder Abuse and Neglect

  1. Elder Abuse and Neglect
You are a charge nurse in the emergency department. An elderly woman arrives with bruises all over the upper torso of her body and down her left arm. She was brought in by her grandson who resides with her, and the grandson shares that he brought her in to be checked out because he thinks she may have fractured her wrist when she fell on the last step going down in the cellar to do laundry. He appears extremely stressed and states that his grandmother suffers from Alzheimer disease. When you put the patient’s name and demographic information into the electronic medical record it indicates that this patient was in 2 weeks prior with a concussion.

    Instructions:

    1. Read the scenario above and answer the following question:
      1. How would you assess the situation?
      2. What nursing interventions would you utilize to help both the elderly woman and the grandson?
    2. Your paper should be:

TALENT MANAGEMENT

TALENT MANAGEMENT

Assignment Overview-Please Attachment for Example Image

Two primary outcomes of HR management are recruitment and selection. Through HR planning, managers anticipate the future supply of and demand for employees and the nature of workforce issues, including the retention of employees. These factors are used when recruiting applicants for job openings. Being able to recruit and retain the “best” employees has been a difficult task for HR managers. The “best” employees are not necessarily the most qualified individuals (e.g., those with the most education or work experience); they are the individuals who provide value to an organization, who complement the organization, who understand and embrace the organization’s mission, and who fit the culture of the organization. This quandary is a result of a number of different variables such as job satisfaction, organizational commitment, job performance, and motivation.

Furthermore, contemporary HR managers also are tasked with assisting with peak performance initiatives because of escalated global demands and competition.

Homework Case Assignment

Go to the Occupational Outlook Handbook, latest edition (https://www.bls.gov/ooh/ ) and read about a Human Resource Manager generalist position. [A generalist is someone who covers many HR functions, as opposed to a specialist who might be responsible for only one or two functions or sub-functions of HRM.] Assume that this is a position you as an HR consultant need to fill for a client in your home state. In this assignment address each of the following:

  1. Develop/include a job announcement/ad to fill the opening. (1 page or 1-3 slides)
  2. Identify and explain your advertising and recruiting strategies to find candidates qualified for this opening for your client company. (1 page or 2 slides)
  3. Go to a website such as Indeed.com and locate a job announcement for the same position. Alternatively, you can meet with your company’s HRM and review a similar job announcement. Compare the obtained announcement with the one you created and write about the differences you see. Which ones are in the announcement you obtained but not in your created announcement? Which ones are in your announcement but not in the one you found? Would you change your announcement to incorporate any of the differences that you noted being on one announcement but not in the other? Why or why not? (1 page or 1-4 slides)
  4. Describe which recruiting and selection processes you will use to choose the best person for the job. (1 page or 1-2 slides)
  5. Create a recruitment flyer that you could put on your company website. Be creative with this requirement. (Match the flyer design to the company’s industry.)
  6. Support your work by using high-quality references (at least one that is found in the Trident Online Library. High-quality references come from peer-reviewed academic journals.

You have a choice: Your submission can be a 5-page Word document (not counting the cover page or the reference page), a 4- to 5-minute video, or a 10+ slide presentation (not counting the cover slide or reference slide page), with voiceover or added speaker comments on the notes section of each slide.

Proofread your work and make sure it is as professional-appearing as possible. If you are submitting a PowerPoint presentation, remember that people today do not want to watch a PowerPoint presentation with slides that are full of words. They want to see no more than 5 words and a picture on every slide. All those words you want to put on the slides belong only on the note pages. A best practice is to insert one short bullet point on each slide. If you want multiple very short bullet points on one slide, then transition each bullet point in one at a time (although recent studies show people do not want to see transitions, either). In addition, the pictures of people should be real people, not clip art, and pictures of employees of the company are preferable. One exception to this “5 words and a picture” rule is with the required job announcement.

In MGT407 (Principles of Human Resource Management) information literacy is assessed at the “introduced level.” Later, in MGT411 (Advanced Topics in Human Resource Management), you will be assessed for information literacy at the reinforced level, and finally in MGT491 (Capstone in HRM) at the emphasized level. Information literacy assignments build upon each other and aim to offer you the opportunity to enhance and practice your information literacy skills. In the below diagram, you will notice that to be truly ‘information literate’ requires that you simultaneously develop:

  • * awareness of how you engage with the digital world
  • * how you find meaning in the information you discover
  • * how to articulate what kind of information you require
  • * how to use information ethically
  • * understand the role you can play in the communication in your profession, and
  •   how you evaluate information for credibility and authority.

These are the things I will grade you on in your assignment submission
