Discussion 1: Cardiovascular Alterations

At least once a year, the media report on a seemingly healthy teenage athlete collapsing during a sports game and dying of heart complications. These incidents continue to outline the importance of physical exams and health screenings for teenagers, especially those who play sports. During these health screenings, examiners check for cardiovascular alterations such as heart murmurs because they can be a sign of an underlying heart disorder. Since many heart alterations rarely have symptoms, they are easy to miss if health professionals are not specifically looking for them. Once cardiovascular alterations are identified in patients, it is important to refer them to specialists who can further investigate the cause.

Consider the following scenario:

A 16-year-old male presents for a sports participation examination. He has no significant medical history and no family history suggestive of risk for premature cardiac death. The patient is examined while sitting slightly recumbent on the exam table and the advanced practice nurse appreciates a grade II/VI systolic murmur heard loudest at the apex of the heart. Other physical findings are within normal limits, the patient denies any cardiovascular symptoms, and a neuromuscular examination is within normal limits. He is cleared with no activity restriction. Later in the season he collapses on the field and dies.

Cardiovascular Alterations·         Review the scenario provided, as well as Chapter 24 in the Huether and McCance text. Consider how you would diagnose and prescribe treatment for the patient.

·         Select one of the following patient factors: genetics, ethnicity, or behavior. Reflect on how the factor you selected might impact diagnosis and prescription of treatment for the patient in the scenario.

In a short essay (500-750 words), answer the Question at the end of Case Study 1. Cite references to support your positions.

Disseminating Results
Take a minute to think about the health-promotion and disease-prevention issue you chose to focus on at the outset of this course. Now bring to mind the problem and target population you identified. The program design and evaluation planning you have engaged in throughout this course have served an important purpose—not only of furthering your own knowledge and expertise, but of contributing toward an understanding of these significant topics and furthering collective progress toward important goals.
As a DNP-prepared nurse, you have an ethical and professional obligation to disseminate findings. This Discussion seeks to facilitate your professional growth by asking you to assess and reflect on the success of your program design, the challenges you have encountered, and the ethical considerations that may warrant additional attention.
To prepare:

• How has your understanding of the      national health-promotion and disease-prevention issue and your selected      problem and target population developed over the past several weeks? How      has your perception changed, if at all?
• Critically evaluate the pros and cons      of the program you have designed. Based on what you have learned in this      course, as well as your professional experiences, do you think your      program would address the problem for the target population? Would the      benefits outweigh the costs?
• Examine the ethical implications of      the program you have designed. What questions, concerns, and/or insights      do you have?
• As a nurse engaged in advance      practice, which venues would be viable for disseminating the results of      your program design and evaluation? What audience would be most interested      in these findings? Identify at least one professional journal to which you      could publish the results of your program design and evaluation, as well      as at least one other venue for disseminating results.

It’s time to describe the monitoring tools for your two compliance plans in a way that all employees will understand at a large medical facility where you are the Compliance Officer.

Your assignment is to write a 2-3 page section describing the monitoring tools for each compliance plan. (That means a total of 4-6 pages for the two plans; you can write them in one Word document.)

Monitoring tools you should cover for each plan should relate to the policies and procedures you developed in previous modules.

You chose policies/procedures under the key compliance areas of Compliance Standards, High-Level Responsibility, Education, Communication, Monitoring/Auditing (for Safety), Enforcement/Discipline, and Response/Prevention. (Check them out if you forget! Remember, you may have written about different policies/procedures for the two different compliance plans.)

Reminder: you are required to include research references throughout your compliance plan – for a total of 12 references for the whole project. That means one or two references per each monitoring tools section for your two compliance plans.
