The unapproved use of approved drugs, also called off-label use, with children is quite common. This is because pediatric dosage guidelines are typically unavailable since very few drugs have been specifically researched and tested with children.

When treating children, prescribers often adjust dosages approved for adults to accommodate a child’s weight. However, children are not just “smaller” adults. Adults and children process and respond to drugs differently in their absorption, distribution, metabolism, and excretion. Children even respond differently during stages from infancy to adolescence. This poses potential safety concerns when prescribing drugs to pediatric patients. As an advanced practice nurse, you have to be aware of safety implications of the off-label use of drugs with this patient group.

To prepare:
  • Review the Bazzano et al. and Mayhew articles in the Learning Resources. Reflect on situations in which children should be prescribed drugs for off-label use.
  • Think about strategies to make the off-label use and dosage of drugs safer for children from infancy to adolescence. Consider specific off-label drugs that you think require extra care and attention when used in pediatrics.

RN-to-BSN program

Throughout the RN-to-BSN program, students are required to participate in scholarly activities outside of clinical practice or professional practice. Examples of scholarly activities include attending conferences, seminars, journal club, grand rounds, morbidity and mortality meetings, interdisciplinary committees, quality improvement committees, and any other opportunities available at your site, within your community, or nationally.  You are required to post one scholarly activity while you are in the BSN program, which should be documented by the end of this course. Consider the following scenario:

The psychiatric mental health nurse practitioner is working for a community health clinic that offers state mandated “intensive outpatient rehabilitation” services for individuals who face incarceration for driving while under the influence. Mr. Smith, a 24-year-old male patient, shows up for his initial intake appointment and says, “Look, this sounds like it’s going to be an AA meeting. Can’t I just go there? I only had a couple of drinks. I don’t know why the judge was such a jerk.”

Clients with addictive disorders may present for therapy for various reasons. Some, like Mr. Smith, attend because it is court mandated. Some attend because family or friends have pushed them to do so, while others may attend because life has become unmanageable. Regardless of clients’ reasons for presenting for therapy, to reach these clients and effectively treat them, you must be able to break down barriers, establish relationships, and help them see the benefits of this therapeutic approach.

This week students will be completing a narrated power point to discuss one of the following cases.

First, students should pick one of the ethical cases above and create a power point presentation to address the following:

  1. What are the facts of the case? This should include: what do we need to know, who is involved in the situation, where does the ethical situation take place, and when does it occur? (3-4 slides)
  2. What is the precise ethical issue in regards to autonomy, nonmaleficence, beneficence, fidelity, and justice? (5-6 slides)
  3. Identify the major principles, rules, and values of the case.  Values are sets of beliefs about good and bad, right and wrong, and about many other aspects of living and interacting in the society with others. A principle is a personal rule that governs personal behavior. A rule is generally imposed by a figure of authority, and used to guide and govern people. (3-5 slides)
  4. Is there legal ground for this case, state what they are? Who is at fault? What legal action should be taken? (2-4 slides)
  5. Are there alternative to the actions completed in this case by the parties involved? What could have been done to prevent the outcome of the case? (3-6 slides)
  6. If you were a member of the ethics committee at the facility the event happened, what action would you take in this case? (2-4 slides)
  7. As a healthcare professional, how did the outcome of the case make you feel? Does it align with your personal values and beliefs? Would caring for this patient be difficult? How do you separate your personal beliefs when completing your job? (4-6 slides)

For the presentation, insure information is referenced and cited in your slides. The presentation should start with a title slide and end with a reference slides. At least 3 references are required for this assignment. If you include pictures, your pictures should also be referenced and cited.
