patient population

Review the Topic Materials and the work completed in NRS-433V to   formulate a PICOT statement for your capstone project.

A PICOT starts with a designated patient population in a particular   clinical area and identifies clinical problems or issues that arise   from clinical care. The intervention should be an independent,   specified nursing change intervention. The intervention cannot require   a provider prescription. Include a comparison to a patient population   not currently receiving the intervention, and specify the timeframe   needed to implement the change process.

Formulate a PICOT statement using the PICOT format provided in the   assigned readings. The PICOT statement will provide a framework for   your capstone project.

In a paper of 500-750 words, clearly identify the clinical problem   and how it can result in a positive patient outcome.

Make sure to address the following on the PICOT statement:

  1. Evidence-Based Solution
  2. Nursing Intervention
  3. Patient Care
  4. Health Care Agency
  5. Nursing   Practice

research design.

I need to answer to this post. Citation, APA and reference

  1. What is the difference in attitudes of male and female college students toward condoms?
  • Identify an appropriate research design.

A research design that would work for this question is exploratory-descriptive qualitative research. This question promotes a potential problem of condom use where knowledge of the use may need to be addressed based on the attitudes of male versus female college students (Gray, Grove & Sutherland, 2017).

  • Discuss the strengths and weaknesses of the design.

One weakness of this design is that you may get a large array of answers about attitude to this question that may be hard to classify. What if there is no opinion on the use of condoms, where would you classify that answer? This can also be a strength if the perception of condom use becomes something obvious such as access to condoms. Also, there is no way to tell if the college students are telling the truth due to the stigma of unprotected sex. Should culture, age and sexual orientation of the students being surveyed and also be taken into account and will these determinant factors skew the results?

  • Provide a rationale for the design you selected

“Exploratory-descriptive qualitative researchers identify a specific lack of knowledge that can be addressed only through seeking the viewpoints of the people most affected” (Gray, Grove & Sutherland, 2017, p.69). This study fits best with this design due to the researcher finding value in the participants perspective and how they can then form interventions to educate the college students based on their findings (Gray, Grove & Sutherland, 2017).

  1. What is the relationship between alcohol and breast cancer?

  Identify an appropriate research design.

An appropriate research design for this research question would be descriptive correlational design since you are measuring the strength of the affiliation between variables (alcohol and breast cancer). It would be a noninterventional research study describing the relationship between the two variables (Gray, Grove & Sutherland, 2017).

  Discuss the strengths and weaknesses of the design.

The strengths of this study include many research findings that link alcohol and breast cancer to support a trend in the relationship. Weakness in this study includes unknown and uncontrollable variables of the participants from previous studies or current. If you are only asking participants or using research with a relationship between these two variables you are leaving out many other risk factors that contribute to these participants risks for being at a higher probability for cancer; these risk factors include obesity, smoking, diet and exercise.

  Provide a rationale for the design you selected

The rationale to use this design is based on the variables of alcohol and cancer and to be able to predict values of their relationship (Gray, Grove & Sutherland, 2017).

  1. What is the difference between self-efficacy scores in older adults who exercise and the scores of those who do not?

  Identify an appropriate research design.

For this research question I would use comparative descriptive design since you are finding a difference between two things; those older adults that do exercise and those that do not (Gray, Grove & Sutherland, 2017).

  Discuss the strengths and weaknesses of the design.

Weakness of this design are based on where you collect your data. If your subjects are in an active senior independent community compared to those in an assisted living facility you may get results that look beneficial to exercise or not. Also, there would need to be classification on what constitutes as exercise since the participants could have different opinions of this. Strengths of this study would be the self-efficacy scores could be simplistic and easy to compare on a numerical scale.

  Provide a rationale for the design you selected

The rationale for this design is that it would be a quantitative non-interventional comparative research study which is used to compare differences (Gray, Grove & Sutherland, 2017).

sexual exploitation

Question #2

There are so many health indicators and concerns for a teen or woman who is a victim of sexual exploitation.  “In a systematic review of the impact of IPV on sexual health, IPV was consistently associated with sexual risk taking, inconsistent condom use, partner non-monogamy, unplanned pregnancies, induced abortions, sexually transmitted infections and sexual dysfunction”(Chamberlin & Levenson, 2011)  These are just some of the physical health concerns they may have.  There are so many emotional concerns that would be linked to sexual exploitation also.   Post-traumatic stress disorder (PTSD), including flashbacks, nightmares, severe anxiety, and uncontrollable thoughts, Depression, including prolonged sadness, feelings of hopelessness, unexplained crying, weight loss or gain, loss of energy or interest in activities previously enjoyed”(Joyful Heart Foundation, 2019).

Georgia specifically has a state wide domestic violence hotline. “Educational videos on temporary protective orders were distributed to Nurse Mangers in all 159 Georgia Counties and 19 Health Districts to utilize in trainings and seminars.  The tapes, obtained from the Georgia Commission on Family Violence, were designed to increase the nurses’ knowledge of services available to victims of domestic and sexual assault, and to enable them to direct these women to alternatives that can help reduce their exposure to violence.  Designed and developed a tri-fold pocket card (in English (Links to an external site.)Links to an external site. and Spanish (Links to an external site.)Links to an external site.), in collaboration with the Georgia Coalition Against Domestic Violence (GCADV),  that contains information on the signs of domestic violence, safety plans, options available to survivors of domestic violence, and a list community organizations that work with survivors of domestic violence”(DPH, 2018).

In my county specifically I know there is an organization called Community Welcome House, Inc.  This organization helps domestic violence victims.  It provides, “Emergency housing sanctuary in the time of crisis Residents receive assistance with medical care, child care, counseling, financial assistance, vocational training, employment and permanent housing”(Domesticshelters.org, 2019).

Question 1—Domestic Violence

Domestic violence can come in many shapes and forms. In some cases, physical injury can occur, while in other cases psychological abuse, deprivation, intimidation or other types of harm can occur (ACOG, 2012). The American College of Obstetricians and Gynecologists (ACOG) recognizes that routine visits and prenatal visits are an ideal time to assess for domestic violence (ACOG, 2012). Assessing for domestic violence can be done by using simple screening questions. These questions should not be asked in front of the abuser or other individuals. ACOG (2012) recommends using a framing statement and confidentiality statement before asking any questions. The framing statement lets the patient know that questions are being asked because relationships play a large role in health and the confidentiality statement lets the patient know that what she states today will not be told to anyone else unless reporting is required (ACOG, 2012).

Risk Factors

Two risk factors for domestic violence include: low education levels and drug and/or alcohol abuse (Huecker & Smock, 2018). Studies have shown that there is an inverse relationship between education levels and rates of domestic violence (Huecker & Smock, 2018). Men are more likely to perpetrate violence if they have low education and women are more likely to experience intimate partner violence (IPV) if they have a low education level (WHO, 2017). Alcohol and drug use are also risk factors for IPV. Alcohol and drug abuse is associated with an increase in the incidence of domestic violence, likely due to the inability of an impaired person to control violent impulses (Huecker & Smock, 2018).

Clinical Signs

Obtaining a history, screening for IPV, and performing a physical exam can help point to IPV. Huecker and Smock (2018) state the most common injuries involved in IPV are on the head, neck, and face. Defensive injuries may also be present on the forearms (Huecker & Smock, 2018). A full physical exam should also evaluate the skin in areas covered by clothing (Huecker & Smock, 2018). Sexual abuse may be harder to identify physically, depending on the nature of the abuse (Huecker & Smock, 2018). Psychological complaints may include: anxiety, depression, and fatigue (Huecker & Smock, 2018). The patient may also have vague complaints, such as chronic pain, headaches, or chest pain (Huecker & Smock, 2018).

comment1

Every organization has its stakeholders, irrespective of its size, nature, structure and purpose. The stakeholders can be any person or entity, who influence and can be influenced by the organization’s activities. Stakeholders can be categorized as internal and external stakeholders. Those who works with in the organization are known as internal stakeholder. Physicians, nurses and hospital management in my case are internal stakeholders. External stakeholders are those not employed by hospital but they have a say in hospital operations such as Licensing or Accreditation agencies like (Joint Commission), patients, policy makers, labor unions and care givers. Change is often difficult to implement because of inertia and so many competing interests. Certain elements must be in place in an organization for change to take hold: an agreed-on direction for the practice, a functional and effective leadership structure, and a culture that promotes and rewards change.

Comment2

Stakeholder buy-in is the glue that binds all elements of a project together and ensures that the change will actually happen”.( May, 2016). It is important to have the backing of the decision makers/stakeholders both within and outside of an organization to successfully push a change project through because:

  1. They will provide expertise from their wealth of knowledge about current process, historical information and way forward, thereby, bringing value to the project.
  2. They will help reduce or uncover risks involved in change project process in terms of project needs, constraints and plans to mitigate those problems as they arise to increase the chances of project success.
  3. They help to increase the success of the project by speeding up the process, providing resources and creating the support needed to move change forward.
  4. Finally, they have the power and authority to grant the project acceptance. (Schoenhard, 2017).

SOC313 Family Document,

Prior to completing this discussion, read Chapter 9 in the textbook, the SOC313 Family Document, and review the Instructor Guidance.

Our stage of life, intellectual/cognitive abilities, and sociocultural position in life, affect our perspectives and resultant behaviors about a number of conditions including cancer. Consider the information provided in the SOC313 Family Document. Both Ella and Evan have been diagnosed with cancer. Ella has been fighting cancer with complementary and alternative methods with some success for many years. Evan, her grandson, is 10 years old and has recently been diagnosed with leukemia but has not yet begun treatment.

Putting yourself in either Ella or Evan’s place, what might your perspective on your cancer be? Integrate how the stage of life, cognitive abilities, and sociocultural position of your chosen person influences her/his perspective on his/her individual disease.

Guided Response: Read several of your classmates’ posts and respond to two of your peers.  Did your classmates select the same or different individual?  Respond to similarities and differences in your perspective and your classmates’ perspectives.  Taking on the role of either Evan’s mother or Ella’s husband, comment on how your perspective of the disease differs from your classmates’ perspectives. Share any additional insights you have after reading these different perspectives.

Course Outcomes (COs).

Purpose

This week’s graded topics relate to the following Course Outcomes (COs).

  • CO 1 – Utilize prior knowledge of theories and principles of nursing and related disciplines to explain expected client behaviors, while differentiating between normal findings, variations, and abnormalities. (PO 1)
  • CO 2 – Recognize the influence that developmental stages have on physical, psychosocial, cultural, and spiritual functioning. (PO 1)
  • CO 4 – Identify teaching/learning needs from the health history of an individual. (PO 2)

The Assignment

As the school nurse working in a college health clinic, you see many opportunities to promote health. Maria is a 40-year-old Hispanic who is in her second year of nursing school. She complains of a 14-pound weight gain since starting school and is afraid of what this will do to both her appearance and health if the trend continues. After conducting her history, you learn that she is an excellent cook and she and her family love to eat foods that reflect their Hispanic heritage. She is married with two school-age children. She attends class a total of 15 hours per week, plus she must be present for 12 hours of labs and clinical. She maintains the household essentially by herself and does all the shopping, cooking, cleaning, and chauffeuring of the children. She states that she is lucky to get 5 hours of sleep per night, but that is okay with her. She drinks coffee all morning to “keep her going.” She lives 1 hour from campus and commutes each day, and often drinks diet cola to “stay awake.” When asked what she does to relax and de-stress, Maria states she “doesn’t even have time to think about that.”

Maria’s vitals today are as follows; T 98.6, R 20, HR 88, BP 148/90

Using the lesson and text as your guide, answer the following questions.

  1. What additional assessment data (subjective and/or objective) would you like to gather from Maria?
  2. What actual health concerns and risk factors have you identified?
  3. What are some opportunities to promote health and wellness for Maria?
  4. Write one nursing diagnosis for Maria (actual, wellness or risk), based on one of the health concerns or opportunities you have identified. (Please use one of the formulas outlined in the text and lesson!)

Remember to use and credit the textbook or lesson, as well as an outside scholarly source, for full credit.

financial consulting company

Dumpster, Inc. is a financial consulting company that, for a fee, advises clients how to manage and invest their money. Dumpster operates in the fictitious state of New Manitoba and has more than 30 employees and more than 1,000 clients. Last year, the company grossed approximately $3.2 in revenue and its owners netted about $500,000 in profit after paying salaries and expenses.

In January of this year, an internal audit revealed that at least three of Dumpster’s employees were intentionally advising clients to purchase small capitalization stock that the employees held in order to drive up its value. Moreover, the same internal investigation showed two instances of employees embezzling client money and using complex spreadsheets and explanations to hide the embezzlement from the client.

You have been asked to come in an establish a risk assessment and mitigation strategy for Dumpster in order to remedy whatever problems have occurred until now and to try to prevent damage to the company going forward.

Please prepare a preliminary report discussing the following:

1. The role that a risk assessment analysis would have in helping to shape a compliance program within Dumpster that could help shield the company from corporate criminal liability.

2. Discuss the ways in which the risk assessment and compliance strategies might treat different levels of employees differently.

3. Discuss some features that a risk mitigation plan might entail for a company like Dumpster.

4. Discuss the risks that Dumpster could face as a result of some of the above-referenced actions of its employees.

5. Briefly describe the steps that would would recommend for conducting the risk assessment.

While the precise length of your report is left to your discretion, we recommend a response to this assignment that is 2-4 (single spaced) pages long.
