In Milestone Two, you will submit a draft of your final project. In Milestone One of the final project, you have chosen a well-known individual from history or current popular literature, with instructor approval, who has a known physiological—as opposed to psychiatric or psychological—disease or illness.

In Milestone Two, you will analyze that disease or illness using a systematic approach. You will also develop a plan of care for that disease or illness.

Prompt: In your analysis, you will describe the pathophysiology for your selected disease or illness, propose historical explanations for variations in findings (when applicable), and demonstrate use of nursing conceptual models to frame your discussion of adaptation and stressors collectively for system analysis.

Your plan of care should utilize the PIE (planning, intervention, and evaluation) format. The planning (P) section should include at least two short-term and two long-term goals that are most appropriate for your chosen case, and each goal should have measurable criteria and have a hypothetical target date or time. The intervention (I) section should include interventions or nursing actions that directly relate to the selected case’s goals and that are specific in action and frequency. The number of interventions should be appropriate for helping to meet individual goals. Finally, the evaluation (E) section of the plan of care should include proposed measures for determining success of the plan and an evaluation of the potential success of the plan, based on those measures. You may write the plan of care in either narrative format or as a table with appropriate in-text citation used to support the plan of care.

Many factors influence how successfully an EBP Project is implemented and evaluated. As noted in this week’s Learning Resources, using a translation framework can facilitate effective implementation and the evaluation of outcomes.

As you complete Application 4, revisit the evidence-based practice model or framework you selected for your EBP Project (Application 2 [see attached file]). How does the model or framework facilitate the evaluation of outcomes? Would another model or framework better meet the needs of your project?

To prepare for this week’s section of Application 4:

  • Continue      (from this week’s Discussion[ use the discussion you are currently working on for me]) to:
    • Identify       an appropriate method for evaluating your outcomes.
    • Develop       new practice guidelines based on the possible results of the evaluation.
    • Create,       if appropriate, new standards of care that would be based on the new practice guidelines.

This paper should be written in APA format with a minimum of 8 scholarly references. All level 1 & 2 headers must be as in the grading rubric (see attached file). Below are the requirements needed for successful completion of the Windshield Survey Form.

1. Introduction to the Community: Identify the community you will be using for this assignment with the city and state, and provide a brief, one-paragraph description of the community. Your community should be the area where you live or the area surrounding your work setting. The community must include a residential area and be a large enough area to answer the survey questions. Do not include epidemiological or demographic data as this is based on what you observe.

2. Windshield Survey: Assess your community by doing a windshield survey. Information about the components of a windshield survey is located in your textbook, Nies and McEwen (2019). Drive through the area and report your observations by answering the questions on the form. Be sure to include what you observed related to each of these categories, and also include any significant items that are missing in your community because this may be equally important in identifying a community health problem.
