Assignment 1: “Captain of the Ship” Project – Schizophrenia Spectrum and Other Psychotic Disorders

In earlier weeks, you were introduced to the concept of the “captain of the ship.” In this Assignment, you become the “captain of the ship” once again as you provide treatment recommendations and identify medical management, community support resources, and follow-up plans for a client with a schizophrenia spectrum/other psychotic disorder.

To prepare for this Assignment:

• Select an adult or older adult client with a schizophrenia spectrum and other psychotic disorder you have seen in your practicum.

In 3–4 pages, write a treatment plan for your client in which you do the following:

• Describe the HPI and clinical impression for the client.
• Recommend psychopharmacologic treatments and describe specific and therapeutic endpoints for your psychopharmacologic agent. (This should relate to HPI and clinical impression.)
• Recommend psychotherapy choices (individual, family, and group) and specific therapeutic endpoints for your choices.
• Identify medical management needs, including primary care needs, specific to this client.
• Identify community support resources (housing, socioeconomic needs, etc.) and community agencies that are available to assist the client.
• Recommend a plan for follow-up intensity and frequency and collaboration with other providers.

In this written assignment, select one cultural factor such as health beliefs, language, perception of time, environment control, etc. (see textbook reading) and apply it to a selected ethnic group. The paper will include the following:

1. One impact on medication preparation. Explain.
2. Two impacts on medication administration. Explain.
3. Two potential adverse reactions. Explain with rationale.
4. One possible issue in adherence to medication regimen. Explain how this can be overcome.

The paper should be no more than 3 pages. Use APA Editorial Format for all citations and references used.

Attend a committee meeting in your health care organization. If you are not currently employed in a health care setting, you may elect to attend a committee meeting at another company, a community center, a local school, local chamber of commerce or other professional organization.

Observe the interactions between committee members and the process used by the committee to arrive at decisions.

In 500-750 words, describe the function of the committee and the roles of those in attendance. Describe your observations of the interactions between members of the committee and determine whether the process used to arrive at decisions is a form of shared governance.

A minimum of two academic references from credible sources are required for this assignment.

The ethical issues presentation will address an ethical issue associated with the practice of nursing. • Describe the key stakeholders that might be involved in this transition of care and the leadership strategies you would use to engage and influence them.
• Explain how you, as a nurse leader along with your healthcare team, would apply systems thinking when providing a transition of care aligned with the IHI Quadruple Aim framework in order to improve it. Explain the fourth aim and strategy you would use and why.
• Explain how systems thinking would inform your improvement plan for the specific transition of care you sele
