Screening

Purpose

Screening, Brief Intervention, and Referral to Treatment (SBIRT) is an evidence-based practice intervention used to identify, reduce, and prevent problematic use, abuse, and dependence on substances such as tobacco, alcohol, and illicit drugs. The SBIRT model was incited by an Institute of Medicine (IOM) recommendation that called for community-based screening for health-risk behaviors. The purpose of this assignment is to integrate SBIRT into nursing practice to help stimulate positive change at the individual and population-focused level of care.

Course Outcomes

This assignment enables the student to meet the following Course Outcomes (COs).

  • CO3: Plan prevention and population-focused interventions for vulnerable populations using professional clinical judgment and evidence-based practice. (PO4, PO8)
  • CO4: Evaluate the delivery of care for individuals, families, aggregates, and communities based on theories and principles of nursing and related disciplines. (PO1)

Due Date

Submit your completed SBIRT presentation by 11:59 p.m. MT Sunday of Week 6.

Points:

225 points

Background information

SBIRT consists of three major components.

1. Screening—A healthcare professional assesses a patient for concerning behaviors using standardized screening tools. Screening can occur in any healthcare setting.

2. Brief Intervention—A healthcare professional engages a patient in a short conversation, providing feedback and advice regarding concerning behaviors.

3. Referral to Treatment—A healthcare professional provides a referral to resources and/or treatment options and additional services available (Substance Abuse and Mental Health Services Administration [SAMHSA], n.d.)

patient population

Formulate a PICOT statement for your capstone project.

A PICOT starts with a designated patient population in a particular   clinical area and identifies clinical problems or issues that arise   from clinical care. The intervention should be an independent,   specified nursing change intervention. The intervention cannot require   a provider prescription. Include a comparison to a patient population   not currently receiving the intervention, and specify the timeframe   needed to implement the change process.

Formulate a PICOT statement using the PICOT format provided in the   assigned readings. The PICOT statement will provide a framework for   your capstone project.

In a paper of 500-750 words, clearly identify the clinical problem   and how it can result in a positive patient outcome.

Make sure to address the following on the PICOT statement:

  1. Evidence-Based Solution
  2. Nursing Intervention
  3. Patient Care
  4. Health Care Agency
  5. Nursing   Practice

Prepare this assignment according to the guidelines found in the APA   Style Guide. An abstract is not required.

This assignment uses a rubric.

You are required to submit this assignment to LopesWrite. Please refer to the directions in the Student Success Center.

bowel/bladder function

A 70-year-old female is admitted to your unit for changes in bowel/bladder function. Her initial lab work was WNL. Her baseline vital signs were BP 124/62, HR 84, RR 20, Temp 98.4.  The MD performed a bowel resection after it was determined that the pt has colon cancer. Post-operatively the pt is taken to a medical/surgical unit. She has an IV of NS at 100ml/hr., and an NGT to intermittent suction, a foley catheter, and a midline abdominal dressing that is dry and intact.

You are assigned the patient post-op day one. Her VS @ 0700, BP 100/60, HR 104, RR 24, Temp. 99.2 orally. the pt is alert and oriented; her color is pale. the NGT is draining brownish-green mucous, the abdominal dressing is D/I, and the IV is at 100 ml/hr. The pt says, “I’m so tired and I feel so nauseous, and it hurts to move.”

The physician ordered the pt to be out of bed. When sitting at the bedside at 1000, the pt states, “I don’t feel very well.” after waiting 30 seconds, the nurse assists the pt to the chair. The pt becomes dizzy and her bp is 90/64, HR 114, and the urine output has been 70 ml since 0600.

1) What would you do first with this pt? List 3 priority interventions you would implement at this time?

2) List 2 categories of medications that you may be administering to this pt and give an example in each category? (remember, the pt has an ng tube)

3) State 2 priority nursing diagnoses?A 70-year-old female is admitted to your unit for changes in bowel/bladder function. Her initial lab work was WNL. Pediatric Bipolar Depression Disorder Debate

Daatiscussion: Pediatric Bipolar Depression Disorder Debate

I WAS CHOOSE FOR SUPPORT THE diagnosing pediatric bipolar depression disorder. NOT AGAINST.

NEEDS 3 EDUCATIONAL APA REFERENCE WITH CITATION.

Some debate in the literature exists specific to whether or not bipolar disorder can be diagnosed in childhood. While some have anecdotally argued that it is not possible for children to develop bipolar disorder (as normal features of childhood confound the diagnosis), other sources argue that pediatric bipolar disorder is a fact.

In this Discussion, you engage in a debate as to whether pediatric bipolar disorder is possible to diagnose.

Learning Objectives

Students will:
  • Evaluate diagnosis of pediatric bipolar depression disorder
  • Analyze consequences to diagnosing/failing to diagnose pediatric bipolar depression disorder

To Prepare for the Discussion:

CHARTING A COURSE FOR CONFLICT RESOLUTION

CASE: CHARTING A COURSE FOR CONFLICT RESOLUTION—“IT’S A POLICY”

Read the case below from Chapter 10 in your textbook:

Background
The setting is an 82-bed hospital located in a small city. One day an employee of the maintenance department asked the supervisor, George Mann, for an hour or two off to take care of some personal business. Mann agreed, and he asked the employee to stop at the garden equipment dealership and buy several small lawnmower parts that the department required. While transacting business at a local bank, the employee was seen by Sally Carter, the supervisor of both human resources and payroll, who was in the bank on hospital business. Carter asked the employee what he was doing there and was told the visit was personal. On returning to the hospital, Sally Carter examined the employee’s time card. The employee had not punched out to indicate when he had left the hospital. Carter noted the time the employee returned, and after the normal working day she marked the card to indicate an absence of 2 hours on personal business. Carter advised the chief executive officer (CEO), Jane Arnold, of what she had done, citing a long-standing policy (in their dusty, and some would say infrequently used, policy manual) requiring an employee to punch out when leaving the premises on personal business. The CEO agreed with Sally Carter’s action. Carter advised Mann of the action and stated that the employee would not be paid for the 2 hours he was gone. Mann was angry. He said he had told the employee not to punch out because he had asked him to pick up some parts on his trip; however, he conceded that the employee’s personal business was probably the greater part of the trip. Carter replied that Mann had no business doing what he had done and that it was his—Mann’s—poor management that had caused the employee to suffer. Mann appealed to the CEO to reopen the matter based on his claim that there was an important side to the story that she had not yet heard. Jane Arnold agreed to hear both managers state their position.

Present a 12 – 15 slide PowerPoint answering the case study questions. The number of slides exclude the title and reference pages. Substantial Speaker Notes are required throughout the entire presentation.  Below you will find articles with more info on speaker notes.

  1. Develop the argument you would be advancing if you were in George Mann’s position.
  2. In similar fashion, thoroughly develop the argument you would advance if you were in Sally Carter’s position.
  3. Assuming the position of the CEO, Jane Arnold, render a decision. (Document your decision in whatever detail may be necessary, complete with explanation of why you decided in this fashion)
  4. Based on your responses to Questions 1 to 3, outline whatever steps—policy changes, guidelines, payroll requirements, or something else—you believe should be considered to minimize the chances of similar conflict in the future.

Reference your readings and include a minimum of 5 peer-reviewed, scholarly, or similar articles.

Format your PowerPoint according to APA guidelines.

summary of six of your articles

Submit a summary of six of your articles on the discussion board. Discuss one strength and one weakness to each of these six articles on why the article may or may not provide sufficient evidence for your practice change.
