Dilemma is the word I have chosen to describe this situation. Unfortunately, I think this is a very common occurrence within this setting for nurse practitioners. The practitioner in this scenario is faced with having integrity in treating the patient properly and seeing other patients and maintaining volume in order to be compensated financially. When it comes to nurse staffing to patient ratio, one article says that it is important to have a balance in order to provide the best patient outcomes (Clarke & Donaldson, 2008). Another article talked about how healthcare has incongruent goals. These goals include improving patient outcomes while providing patient-centered care and keeping costs down (Judge-Ellis & Wilson, 2017). This is exactly what this scenario shows.
My Experience with this Situation
       I have felt in a similar situation on a smaller scale. When our unit became increasingly busy and our staffing was down I had to take on an additional patient and,at the time, I was not comfortable with the load. I felt that I was not equipped to give the patients the best care and safety required. Unfortunately, our unit was not in a position to get additional help, especially so quickly, and I had to figure it out. I was able to care for the patients as needed and got some help. The amount of stress and even fear this added to my work was worrisome. This is not a position that was comfortable and I would think being in this given scenario would be worse. I would hate to have my work integrity and my finances go head to head. I would like to think I would care for the patients and I am sure at times I would and at others the financial aspect would guide my decision making.

The word metaphor I chose for this scenario is “actuality”, because a similar scenario likely unfolds in primary care offices around the country on a daily basis. During my most recent clinical rotation, it was expected that all providers schedule a patient every 15 minutes, and remain timely. This is common practice throughout primary care in the United States. Primary care institutions across the U.S strive to create value for their consumers (Budrevičiūtė et al, 2018), but at what cost? During my rotation, I struggled to identify what needs of the patient were most important to them. Often, I felt as though the patients could benefit from a little extra time with the provider.
            As nurses, it is engrained in our being to put the patient first, and address all needs. Being a nurse practitioner does not change our inherent desire to fix all of our patient’s problems. However, this does set us aside from other providers such as MDs, DOs, and PAs. From the time we are in nursing school to now – we are taught to address the whole patient. So, how I would address this issue? I would prioritize the patient’s most pressing health-care needs. I would follow this patient on a regular basis, even weekly if needed to put him on the right track.

What are enlarged montgomery glands? What do they look like?

The Areolar glands, also known as Montgomery’s glands, are sebaceous glands in the areola.
Enlarged ones happen with pregnancy. They produce an oil-like substance that makes it easier for the breast tissue to deal with the hard sucking on the breasts that the newborn will do.
If you look at the breast, you will see a darker colored area around the nipple. This dark area is the areola. The glands look a little like big pimples.
Sometimes they get clogged but don’t try to ‘pop’ them. You can cause an infection. Massage, hot compresses, salt water compresses, and rubbing alcohol will help. But no squeezing!
