Prostate Cancer Screening for African American Men

Problem and a Population: Prostate Cancer Screening for African American Men
In todays healthcare racial disparities in prostate cancer incidence and mortality are a national concern (Husaini et al. 2008). African American men unfortunately have the highest incidence of disease rates and are close to 2.5 times more likely to die from prostate cancer when compared to Caucasian men (American Cancer Society 2013). For African American males the risk of developing prostate cancer is now 1 in 5 (American Cancer Society 2013; Centers for Disease Control and Prevention [CDC] 2014). In addition African American men also have a higher rate of death due to prostate cancer than any other ethnic groups (CDC 2014). This fact is evident considering the life expectancy for African American men is several years shorter than other ethnic groups (Arias 2011). According to American Cancer Society (2013) prostate cancer exhibits the most pronounced racial disparity of all cancers in the United States. In response to increase prevalence of prostate cancer among African American men a pilot program in a church-based setting will be developed to increase knowledge about prostate cancer screening by decreasing the health disparity that exists among this vulnerable group of men.
TITLE: Application 1: Initial Analysis for Program Planning
APA 6th Edition 5 references scholarly peer review articlesA 3 PAGE paper that addresses the following:Identification of a Problem and a PopulationIdentify a problem and a target population.
(Note: Your problem should be clearly developed and related to a health-promotion and disease-prevention issue of national significance. You should select a target population so that your program can be designed to create maximum yet realistic impact at the aggregate level.)
Using a Theory or Model
Analyze a theory or model that is relevant to your selected problem. Explain why this is the most appropriate model for you to use with this problem.Literature Review
Assess the literature as it relates to your problem. Use evidence from the literature to support your response to the following question: Why is it important to assist this population to resolve the selected problem?
Needs Assessment
Describe an appropriate approach to conducting a needs assessment for your problem.Identify method(s) of data collection.Assess potential challenges and propose alternatives/solutions.TITLE: Application 2: Program Design ElementsAPA 6th Edition 5 references scholarly peer review articlesA 3 PAGE paper plus accompanying documents that addresses the following:Stakeholder Involvement in Developing Mission Statement Goals and Objectives
Explain why it is important for representatives of your target population to be involved in developing the goals and objectives for the program.Identify which stakeholders you would involve in the planning process and discuss two or more strategies for facilitating their involvement.
Program Design
Create a visual representation of your program design (e.g. a table or graph) that includes the programs mission statement goal(s) objectives and activities to meet the objectives.Develop a Gantt chart with time lines for implementing all activities that will assist in meeting each of your program objectives.
TITLE: Application 3: Program Budget and Financial AnalysisAPA 6th Edition 5 references scholarly peer review articlesA 3 page paper that addresses the following:
Budget
Develop a simple revenue and expense budget for 6 months from start-up and develop a break-even analysis.Articulate a justification for the revenue and expenses indicating where the funds are coming from to support the project and what resources the expenses are related to.
Explain any variance from the budget that is not budget neutral.
Financial Analysis
Articulate whether the monies put into the program will result in positive health outcomes for the population (resulting from your application of the use of cost-effectiveness cost-utility or cost-benefit to the program design process).* Include tables or graphs in your paper to illustrate budget informationTITLE: Application 4: Evaluation PlanningA 3 page paper that addresses the following:
Evaluation Methods
Identify an evaluation theory or model that is aligned to your program goal(s) and objectives.Exhibit a performance measurement monitoring and evaluation time line that:o Demonstrates the appropriate use of performance measurement monitoring and summative evaluationo Distinguishes between the long-term effects of impact evaluation versus short and intermediate health outcomes as a result of the implementation of the program
Evaluation Plan
Develop an evaluation plan that includes goals objectives and activities. Specify the type of data needed.Add your evaluation plan to the visual representation (e.g. table or graph) of your program plan design.Add time line information for the evaluation plan to your Gantt chart.Note:In addition to your paper be sure to submit the following (which may be contained in a separate document):An updated version of the visual representation (e.g. table or graph) of your program design that includes your evaluation plan.An updated version of your Gantt chart that includes your evaluation plan.
