Using Process Maps for Quality Assessment and Improvement

Given the synergistic relationship between process and outcomes, leader-managers and other team members need to evaluate processes related to care delivery, customer service, and other matters in order to diagnose problems and effectively improve quality.

A process map is a visual representation that illustrates activities and practices as they occur, which serves as a valuable resource for identifying trouble spots. It is important to note that process maps can also be used for evaluation after an intervention has been implemented. For instance, looking at a process map following an intervention may help leader-managers to note discrepancies between what was planned and what is actually being practiced, or to see that a process has in fact improved, even if the outcomes have not met the targets.

In this Discussion, you examine how process mapping could be applied to your Course Project.

To prepare:

Review the Learning Resources, focusing on the use of process maps for improving quality and safety.
Bring to mind your work on your Course Project thus far. How could you use a process map to diagnose problems and inform strategies for improvement? In addition, think about how using a process map could inform and enhance your data collection efforts.

No one can predict the future, but accountants and financial managers must try and do exactly that!  By examining net revenue, costs, and cash flow, you can get a clearer picture of what to expect in your organization’s (or one with which you are familiar) fiscal future. Using these metrics to look forward will enable you to more effectively plan budgets that accomplish organizational goals.  When developing a budget, what variables do you have to take into account? In health care organizations, two of the largest groups of factors that you must consider are first, volume, and second, staffing and supply. The number of patients and tests performed each day, as well as employees and their pay rates are all crucial pieces of information when determining a budget.  In this Assignment, you address five scenarios: net revenue, fixed and variable costs, cash flow, volume budget, and staffing and supplies budget.   Note: For those Assignments in this course that require you to perform calculations you must:  •Use the Excel spreadsheet template for the Week 5 assignment.

You obviously want the highest possible price, but as you enter the negotiations, what is the lowest possible price you would be willing to accept from this payer?  Hint: Calculate the variable cost.  Scenario 3: Cash Flow Scenario  Your new business venture will begin operation on July 1, 20X2. You will hire staff effective January 1, 20X2 with a cost of $40,000 per month. You know from experience that collections lag billing by 3 months (in other words, once you bill for a service, you must wait 90 days for the payment to be received.) Your business volume is projected to be as follows:   Question: If you have $380,000 of cash on hand on January 1, 20X2, how much cash will you have at the end of June 20X3?  Assume a 100% collection rate.  Scenario 4: Volume Budget Scenario  You manage lab services in a large hospital. You have the following data on both the hospital’s budgeted patient days and visits for budget year 20XX along with the ratio of lab tests to patient days or visits.  Question: Based on this raw data provided , how many lab tests would you anticipate for the coming budget year? If each test is priced at $20.00, how much gross revenue would you budget? The following spending data is available for the period January 1 to March 31, 20X7 during which time procedure volume amounted to 3,240. CHINESE CASE STUDY #1
An elderly, Asian-looking man is admitted
to the emergency room with chest pain;
difficulty breathing; diaphoresis; vomiting; pale, cold, clammy skin; and apprehension.
Three people, speaking a mixture of Englis
h and a foreign language to one another,
accompany him. The nurse tries to speak E
nglish with the man, but he cannot understand
anything she says. Accompanying the elderly
man are two women (one elderly and very
upset and one younger who stands back from the other three people) and one younger
man.
The younger man states that the elderly ma
n, whose name is Li Ying Bin, is his
father; the elderly woman, his mother; and
the younger woman, his wife. The son serves
as the translator. Li Ying Bin comes from a sm
all village close to Beijing. He is 68 years
old, and he has been suffering with minor ch
est pain and has had trouble breathing for 2
days. He is placed in the cardiac room, and the assessment continues.
Mr. Li is on vacation, visiting his son and
daughter-in-law in the city. His son and
daughter-in-law have been married for only 1 ye
ar, but the son has lived in the West for 7
years. Mr. Li’s daughter-in-law looks Chin
ese but was born in the United States. She
does not speak very many words of Chinese.
Further physical assessment reveals that Mr
. Li has a history of “heart problems,”
but the son does not know much about them. Mr. Li had been to the hospital in Beijing
but did not like the care he
received there and returned home as soon as possible. He goes
to the local clinic periodically when the pa
in increases, and the h
ealth-care provider in
China used traditional Chinese medicine,
herbs, and acupuncture. In the past, those

treatments relieved his symptoms.
Medications are ordered to relieve
pain, and Mr. Li undergoes diagnostic
procedures to determine his cardiac status. The
studies reveal that he
did sustain massive
heart damage. Routine interventions ar
e ordered, including heart medications,
anticoagulants, oxygen, intravenous fluids, be
drest, and close mon
itoring. His condition
is stabilized, and he is sent to the cardiac intensive-care unit.
In the cardiac unit, the nurse finds Mrs.
Li covering up Mr. Li until he sweats, and
Mrs. Li argues with the nurse every time
her husband is supposed to dangle his legs. She
complains that he is too cold and brings in
hot herbal beverages for him to drink. She
does not follow the nurse’s and physician’s orde
rs for dietary restrictions, and she begins
to hide her treatments from the staff. Her
son and daughter-in-law tr
y to explain to her
that this is not good, but she continues the
traditional Chinese medicine treatments.
Mr. Li is a very quiet patient. He li
es in bed and never calls for help. He
frequently seems to be meditating and exercisi
ng his arms. When he does talk to his son,
he speaks of the airplane ride and the probl
ems of being so high. He believes that may
have caused his current heart problem. Mr. Li
also wonders if Western food could be bad
for his system. Mr. Li’s condition gradually de
teriorates over the next few days. Nurses
and physicians attempt to tell the family a
bout his condition and po

family will not talk with them about it. Mr. Li dies on the 5th day.

APANESE CASE STUDY #1
This case study is a composite of actual situ
ations. Marianne, who is American, and Ken
Shimizu, who is Japanese, have worked
in Tokyo for over 30 years as Methodist
missionaries. They have annual furloughs and
occasional sabbaticals, during which they
visit relatives and sponsoring
organizations and engage in
continuing education in the
United States. They met as college students
in the United States, and their three grown
children have established their ow
n careers in the United States.
Ken’s 98-year-old mother resides with
Marianne and Ken. She is not Christian
but has always been extremely supportive of
Ken and Marianne’s work. Ken teaches at
a large Christian university, whereas Marian
ne has served in various church-related
positions over the years. As missionaries, they live in subsidized post–World War II
housing near Ken’s university. Marianne has
been a frugal housewife, preparing local
foods in the Japanese style for her family.
Ken, who is nearly 60, recently learned
that he has glaucoma. By the time it
was discovered, he had lost a signific
ant amount of peripheral vision. Although
Marianne delivered all three
children at a Christian hos
pital in Tokyo, she gets her
annual physical examination when visiting rela
tives in the United States. She has never
believed that the Japanese health system is
as proactive as that in the United States.
On her most recent visit to the United
States, Marianne learned that she has
hypertension. Her physician prescribed a medica
tion that is readily available in Japan,
but the physician was concerned about the leve
l of stress in Marianne’s life. Mother
Shimizu is quite confused and requires considerable care, but it is unthinkable for Ken,
the only child, to put his mother in a long-
term-care facility. Even if he would, the

quality of facilities in Japan l
eaves much to be desired. Most of the responsibility for
Mother Shimizu falls on Marianne, in addi
tion to her work. Marianne’s relatives are
urging her to consider placing Mother Shimiz
u in a church-related life-care community
near Marianne’s family in the United Stat
es, where Marianne and Ken would like to
retire. Marianne’s own parents
lived in this facility at the end of their lives. She is
considering these issues
as she returns to Tokyo.

New technology—and the application of existing technology—only appears in healthcare settings after careful and significant research. The stakes are high, and new clinical systems need to offer evidence of positive impact on outcomes or efficiencies.

Nurse informaticists and healthcare leaders formulate clinical system strategies. As these strategies are often based on technology trends, informaticists and others have then benefited from consulting existing research to inform their thinking.

In this Assignment, you will review existing research focused on the application of clinical systems. After reviewing, you will summarize your findings.

To Prepare:

  • Review the Resources and reflect on the impact of clinical systems on outcomes and efficiencies within the context of nursing practice and healthcare delivery.
  • Conduct a search for recent (within the last 5 years) research focused on the application of clinical systems. To be honest, I have never had an interest in creating a professional portfolio before reviewing this material. I thought this was something only actors and models did to showcase their headshots. I had never heard of a Curriculum Vitae. I had a fancy resume with my information types on it, I had even gone as far as had it printed on the extensive paper. Without having completed a portfolio, I understand why I was not selected for some positions I applied for.

I believe I first need to start with writing my Curriculum Vitae, as it states on the Walden Writing Centers Guide, curtail your Curriculum Vitae guide to the position you are applying for. This would go for your entire portfolio. I think the mean of it stays the same, but some minor adjustments to fit the position would be needed, highlight academic and accomplishments that relate to that area vs. another.  Updated your resume is critical, including writing a specific job-related summary, I do not currently have an overview of my resume. Reviewing the articles on Professional Portfolios, my understanding is they are to display your progression and accomplishments along with your advancements and career path. It is to be a paper interview. I believe you have a few options to include academic activities and achievements. A person can list them in the Curriculum Vitae, which seems straight forward. Another option is they can be displayed by awards in a subcategory. A person can create an e-portfolio or a paper portfolio depending on the situation. There are so many options, by being creative, you show some of your personality through your individual collection.

An academic portfolio is beneficial for a new scholar to maintain, as it highlights knowledge gained, support of evidence-based practice, and accomplishments.  The portfolio exhibits not only past education but continued education within intended career field, which is felt to be linked with career development and salary (Casey & Egan, 2013).  Portfolios can be in paper form or through a digital platform.  It is estimated that 60 to 80% of perspective employers conduct online search of each candidate and it is recommended for all practicing profession/students to be aware of the digital presences across all platforms (Hannans & Olivo, 2017).  In this digital age, it is important for everyone to be aware of the content posted or endorsed as it can be a direct reflection upon the person posting and cause limitations within a career if deemed inappropriate from perspective employers.  The portfolio should demonstrate a commitment to continued competency within profession, should be unique to the person, clear and orderly, and should grow with the person across the lifespan of their career (Smith, 2011).  It is important for each nursing professional to develop strategies for portfolio development and digital content.

Strategies Identified

The first strategy for portfolio development would be platform consideration.  For students, some schools provide platforms such as Portfolium and Pathbrite, whereas, other social media platforms such as LinkedIn, Careerbuilder, or Monster.com (Hannans & Olivo, 2017).  Social media platforms allow for the professional or student to not only highlight their accomplishments but connects them with other like professionals and opens a network of job opportunities.  Again, it is important to remain aware of what digital content is being released as it can be a depiction of the posting individual.  Some employers still require a resume or paper copies of career achievements, but it is good practice for networking to have a digital presence as well.

The second strategy for building a professional portfolio is remaining committed to professional development, advancement, and continuing education.  This commitment enhances the portfolio development over the years but shows employers/network of colleagues, professional growth.  Competency of nursing abilities remains the fundamental bases for quality patient care and positive outcomes (Karami, Farokhzadian, & Foroughameri, 2017).  In the development of professional competencies, an organizational commitment develops which propels each professional forward into activities outside of their normal requirements (Karami, Farokhzadian, & Foroughameri, 2017).  This strategy will continue to elevate each professional their commitment to professional growth increases.

A well-developed portfolio is a chronological, visual representation of your professional growth. The portfolio does not replace your curriculum vitae (CV) or resume, rather it is the supporting documentation that validates what you have stated on your CV. As a nurse scholar, successful branding is crucial to success in this industry and that is what will set you apart from your competitors. For this discussion, I have identified two strategies that will provide further details look at my accomplishments,

First, strategy is to make sure I have a current resume to showcase my education, achievements and to provide some background information. In the article” The use of professional portfolios for career enhancement”, Casey and Egan stated that it is important to include: “Biographical information, Educational background, Employment history, and professional qualification certificates” (Casey & Egan, 2013). This information provides an overview of what I currently have in my resume which includes my education, background and accomplishments which is a great way of introducing yourself to the people. Therefore, another key strategy is to plan on how and what would be the best way to layout all these pieces of documents to achieve the best display of your hard work in an organized and accurate manner.

Second strategy is through an online ePortfolio which is an online presentation or representation of my academic and professional experiences and achievements. An eportfolio is a way to show to potential employers how you want to be viewed professionally. “Rather than just a retrospective account of continuing professional development activities, a portfolio can be used as a vehicle for engaging in self-assessment and personal development planning. This online portfolio allows any employer to learn about the candidate’s

accomplishments whether they are a fit for the position before the face to face interview. My eportfolio helps me in demonstrating my wealth of experience and help substantiate what I am saying during an interview.
