In order to undertake this 1500 word essay you will need to research the topic using current and relevant peer reviewed literature and review of:  The lecture and tutorial material associated with the relevant topic.  Your knowledge and understanding related to: 1. The physiology and pathophysiology of the primary diagnosis and associated clinical data identified within the chosen case study; 2. Statistics

1-Statistics are extremely important in every research/study conducted and are even important in our everyday lives. Statistics can be used in many ways, some might use it to prove a new procedure should be used due to evidence based research, some might use it to evalute cause and effect, and some might even use it to sway the populations opinion or views on a certain topic. Statistics can absolutely be used for the betterment of the community or it is be used negatively (whether it is intentional or not). There are many different examples of statistics being used inappropriately. Statisticshowto gives great examples of statistics being used in a negative way:

  1. “Anyone      remember Colgate’s claim that 80% of dentists recommended the      brand? You won’t be seeing that slogan again, at least not in the UK.      Consumers were led to believe that 80% of dentists recommended Colgate      while 20% recommended other brands. It turns out that when dentists were      surveyed, they could choose several brands — not just one. So other brands      could be just as popular as Colgate. This completely misleading statistic      was banned by the Advertising Standards Authority”
  2. “In      2009 and 2010, Reebok made the following claims about its EasyTone and      RunTone shoes: Lab tests “proved” that the shoes work “your hamstrings and      calves up to 11% harder and tone your butt up to 28% more than regular      sneakers … just by walking!”. The figures turned out to be complete      garbage. The FTC stated that Reebok needed to pay a settlement of $25      million for deceptive advertising.”
  3. “Perhaps      the most famous case ever of misleading statistics in the news is the case      of Sally Clark, who was convicted of murdering her children. She was      freed after it was found the statistics used in her murder trial were      completely wrong.”

2-The misuse of statistics is common in society trying to influence or drive certain agendas in many fields such as healthcare, sports, politics, advertisement and many others. For the Prime Minister Benjamin to classify statistics as a form of lie, he was not further from the truth, but there is a pre-condition, which should address how researchers are applying the given statistics. When done openly and without bias, while avoiding errors, would increase, the validity and accuracy of conclusions made from the study.

 

3-Statistics involves mathematical procedures, which involves all the details from conducting a certain research to the conclusion of the research. It starts with raw data collected, compiling of such data, analyzing the data, drawing conclusions before representation. Misuse of statistical data sets happens when researchers and those conducting the study mishandle the data either willingly or unwillingly. Errors made during the process of research end up bringing about mistakes in conclusion while intentional errors are supposed to help spread a certain bias as noted in your hotel advertisement.

Hello Professor and class,

I chose case study #3 of an 80 year old retired army officer living along with type 2 diabetes, who had episodes of hypoglycemia using insulin and OHA’s.  The patient was dependent on an attendant to administer injections and due to irregularity of insulin dose administration this would lead to episodes of hypoglycemia.  The patient was then put on Degludec insulin and over a period of time, the patient’s HgBA1C and fasting blood sugars showed improvement. The patient also had no further episodes of hypoglycemia (Kumar, 2015.  p. 877).

Formulating a clinical question in the PICO (T) format I questioned: Do elderly patients with Type 2 diabetes show improvement with episodes of hypoglycemia, FBS and HgBA1C with Degludec?

According to (Stillwell et al, 2010. p. 59), PICOT is an acronym used for elements of the clinical question.  They are helpful for summarizing research question that explore the effects of therapy (Riva et al, 2012. p. 168).

P- Patient population

I-Intervention or issue of interest

C- Comparison of intervention or interest

O- Outcome of intervention or interest

T- Time it takes for intervention to be effective

(Stillwell et al, 2010. p. 59), Also points out there are 2 types of clinical questions, background and foreground.  Foreground question appear to use questions asked using the PICOT format to determine which interventions are most successful for patient outcomes.

Nursing research strives to understand phenomena that impact health, seeks solutions to problems, tests approaches to improving nursing care, and generates new knowledge to further our profession (CCN, 2017)

My clinical question using the PICOT format:

P- Elderly patients with type 2 diabetes

I- On Degludec and OHA’s

C- On regular insulin and OHA’s

O-Improvement of FBS, HgBA1C and no further episodes of hypoglycemia

T- No time frame measured

In asking the question: Is Degludec effective in improving hypoglycemic episodes the focus of my question would be on the prognosis of treatment.  Are patients having less episodes of hypoglycemia, is disease control with treatment?  According to the case study once the patient had started on Degludec it was noted that there was improvement in FBS and HgBA1C and no further evidence of hypoglycemia.

Case Study Analysis

45-year-old woman presents with chief complaint of 3-day duration of shortness of breath, cough with thick green sputum production, and fevers. Patient has history of COPD with chronic cough but states the cough has gotten much worse and is interfering with her sleep. Sputum is thicker and harder for her to expectorate. CXR reveals flattened diaphragm and increased AP diameter. Auscultation demonstrates hyper resonance and coarse rales and rhonchi throughout all lung fields.

A portfolio is a great way to for nurses to “record and provide evidence of skills, achievements, experience, development and learning, not only for themselves, but for the information and scrutiny of registration boards, employers, managers and peers” (Green, Wyllie, & Jackson, 2014, p. 8). As you collect the artifacts that best showcase your strengths, you begin to track, reflect on, and evaluate your own learning and growth over time.

For this assessment, your ePortfolio will focus on showcasing your growth and accomplishments in the context of the following course competencies:

For each competency, complete the following in your ePortfolio:

  • Create an area in your ePortfolio for the competency and label it with the course competency number or the competency language.
  • Select the two artifacts (assessments) that you feel best showcase your accomplishments or growth relative to the competency and place them in the corresponding ePortfolio area.
  • For each competency, write 2–3 paragraphs detailing the ways in the artifacts illustrate your accomplishments or growth in the competency so reviewers can easily understand your achievements and strengths. Place this in the corresponding ePortfolio area with your chosen artifacts.
