Purpose

The purpose of the Course Project is to give the student the opportunity to apply concepts in transcultural nursing by performing a transcultural nursing assessment.

The purpose of Milestone 1 is to identify an adult who is not a relative who is willing to participate in the transcultural assessment and to secure the instructor’s approval for the assessment. Note: The actual assessment does not occur until Milestone 3. It is not a purpose for this assignment.

Requirements and Guidelines

1. Review the Course Outcomes for this assignment, which are listed above.

2. Review the guidelines for Milestones 1, 2, and 3, paying particular attention to Milestone 1 for this assignment. The guidelines for all of the Milestones are located in Modules on the left-hand navigation menu. 3. Identify a potential individual who has been living in a culture other than the one of origin for at least 2 years. This individual must be born outside the United States. The individual can be a patient, friend, or colleague, such as a physician, or acquaintance, but cannot be a relative. The individual must be living in your community and must be currently receiving or have previously received healthcare services in your community. The individual’s race, ethnicity, language, religion, and culturally based beliefs about healthcare and illness should be quite different from yours. The individual must speak your language. An interpreter is not permitted for the actual assessment, which occurs in Milestone 3 (Week 6).
Note: This is the same individual for whom you will develop verbal assessment questions in Milestone 2 and whom you will ask those questions in Milestone 3 and then reflect on your learning about the Course Project and the assessment.

4. Individuals can come from a variety of settings. Recent immigrants, migrant workers, exchange students, international students, long-term care residents, refugees, coworkers, physicians, patients, employees at local ethnic businesses, religious venues, community centers, and individuals who have recently moved to your community are some suggestions. The individual does not need to be a healthcare worker but must have received healthcare services in the country of origin as well as since arriving in the United States. Work with your instructor prior to submitting Milestone 1. Approval must be provided by your instructor before submitting Milestone 1 (this assignment).

5. Speak with the individual, and obtain permission to conduct an assessment later in the course. Clearly explain that statements made in the assessment will be recorded (written) and submitted to the instructor only. It is not intended for public distribution. Anonymity will be maintained by using first and last initials only. Do not use names in any of your documents.

6. Download the NR394_Individual_Profile_Template (Links to an external site.). Although you are not performing the official assessment until Milestone 3, it will be necessary to obtain some general information about your individual to complete Milestone 1. This information may come from the individual or Internet sources.

In clinical settings, advanced practice nurses often encounter patients with blood disorders such as anemia. Consider the case of a 17-year-old girl who is rushed to the emergency room after suddenly fainting. The girl’s mother reports that her daughter has had difficulty concentrating for the past week, frequently becomes dizzy, and has not been eating normally due to digestion problems. The mother also informs the nurse that their family has a history of anemia. With the family history of anemia, it appears that this is the likely diagnosis. However, in order to properly diagnose and treat the patient, not only must her symptoms and family history be considered, but also factors such as gender, ethnicity, age, and behavior. This poses the question: How do patient factors impact the incidence and prevalence of different types of anemia?

To Prepare

  • Review Chapter 21 in the Huether and McCance text. Reflect on the pathophysiological mechanisms of iron deficiency anemia.
  • Select one of the following types of anemia: pernicious anemia, folate deficiency anemia, sideroblastic anemia, chronic inflammation anemia, or post-hemorrhagic anemia. Identify the pathophysiological mechanisms of the anemia you selected.

Epidemiology in the News: Randomized Trials

According to the Centers for Disease Control (CDC), asthma is a leading chronic illness in children ages 5 to 17, a leading cause of school absenteeism, and the third highest cause of hospitalizations in children younger than 15 (2011). The costs associated with treating this chronic illness are high.

A number of randomized control trials have examined the effect of instituting a home management program for treating asthma in combination with pharmaceuticals. These studies found that, by implementing a structured home management program, the morbidity, severity, and frequency of asthmatic episodes were reduced (Agrawl, Singh, Mathew, & Malhi, 2005). Recent research is also exploring how and when to cut back on levels of medication as asthma episodes become controlled, further reducing the cost of this chronic illness; however, controversy remains over the long-term effects of reducing levels of medication due to the disparateness of asthma (Rogers & Reiberman, 2012).

For this Discussion, you are asked to identify an example of an experimental study design in the popular literature and consider the ethical implications of the randomized control trial design.

The authors of this article explored treatment options for smoking cessation by examining the research literature, including looking at the results of randomized control trials. This article provides an example of how such a literature review can influence practice demonstrate the use and benefits of randomized control trial study designs.

This article examines programs that integrate substance abuse treatment and pregnancy, parenting, or child services. The authors conclude that there is a need for funding of high-quality randomized control trial and improved reporting practices. This article also demonstrates the application of analytic research designs to explore a population health problem.

This research study used a randomized control trial to examine the association between hand washing and reducing influenza. As you review this article, notice how they structured the research study and how they measured the association between hand washing and absenteeism.

This large-scale, randomized clinical trial that began in 1982 was designed to test the effectiveness of aspirin and beta carotene in preventing heart attacks (myocardial infarctions, or MIs) in male physicians aged 40–84. The first phase of the trial, which included more than 22,000 study subjects, demonstrated that low-dose aspirin does, in fact, reduce the risk of a first MI by 44%. This article provides a good example of the types of large-scale studies conducted using epidemiologic principles that lead to improved population health.
