patient-safety issue

Write 8-10 pages in which you identify a major patient-safety issue  within your own organization and use evidence-based best practices and  technology to develop a plan to improve the safety issue.

Quality improvement and patient safety are central to the nursing leadership role.

Central to the nursing leadership role, quality improvement and  patient safety are analyzed from many perspectives. Types of quality  improvement and patient safety programs may range from internal,  organization-based quality improvement team reports to external  benchmarks from The Joint Commission, the Agency for Healthcare Research  and Quality (AHRQ), MAGNET, and numerous other organizations.

A landmark 2001 publication by the Institute of Medicine (IOM)  identified the imperative to focus on quality care and patient safety.  The initiative to create cultures of patient safety and quality care  remain at the forefront of the health care leadership landscape. As a  future nursing leader, you must understand the components and use of  effective tools for successful quality improvement programs within your  practice setting.

In this program, you have also examined the application of  research and information technology in the nursing profession.  Information systems and patient-care technologies have contributed in  many ways to improved patient outcomes; however, safeguards, secure  practices, and ethical standards are necessary to promote and support  data security, patient confidentiality, and regulatory requirements. You  have also investigated the effective use of patient-care technologies,  communication systems, and information systems across the health-illness  continuum.

To help you prepare for this assessment, review the resources you used  and the assessments you completed in BSN-FP4004, BSN-FP4006, BSN-FP4008,  and BSN-FP4016.

Preparation

As you prepare for this assessment, complete the following:

  1. Before you begin, examine your organization’s history of safety  in a specific area and how your organization addresses patient safety  issues. If possible, consult with a key stakeholder in the organization  (such as an administrator) to better understand specific patient-safety  concerns and how the organization is working to resolve the concerns.  This person should also be able to discuss some of the organizational  barriers impacting the patient safety issue.
  2. Next, look at the basic concepts, principles, and practices that  contribute to organizational quality improvement and patient safety.  Review the literature for best practices and how technology might be  used to improve the issue.
  3. Finally, be sure to consider the legal and ethical implications  associated with the safety issue, as well as possible organizational  barriers to change.

Directions

As you construct this assessment, address each point as completely as possible:

  • Describe a patient-safety issue within your organization,  comparing the way your organization addresses the issue with the  concepts, principles, and practices that contribute to quality  improvement and patient safety.
  • Analyze the legal and ethical consequences of not addressing the issue.
  • Recommend evidence-based interventions, including technology, to address the patient-safety issue.
  • Describe strategies to overcome specific organizational barriers to change, based on your knowledge of the organization.

Additional Requirements

  • Format: Include a title page and reference page. Use APA style and formatting.
  • Length: Ensure your completed assessment is 8–10 pages in length, not including the title page and reference page.
  • References: Cite at least five current scholarly or professional resources.
  • Font: Use double-spaced, 12-point, Times New Roman font

Critiquing Quantitative, Qualitative, or Mixed Methods Studies

Application: Critiquing Quantitative, Qualitative, or Mixed Methods Studies

Critiquing the validity and robustness of research featured in journal articles provides a critical foundation for engaging in evidence-based practice. In Weeks 5 and 6, you explored quantitative research designs. In Week 7, you will examine qualitative and mixed methods research designs. For this Assignment, which is due by Day 7 of Week 7, you critique a quantitative and either a qualitative or a mixed methods research study and compare the types of information obtained in each.

To prepare:

  • Select a health topic of interest to you that is relevant to your current area of practice. The topic may be your Course Portfolio Project or a different topic of your choice.

Using the Walden Library, locate two articles in scholarly journals that deal with your portfolio topic:  (does hand washing and appropriate staff dressing among the surgical ward nurses reduce cross infection during patient management?) 1) Select one article that utilizes a quantitative research design and 2)  select a second article  that utilizes either a qualitative OR  a mixed methods design.  These need to be single studies not systematic or integrative reviews (including meta-analysis and metasynthesis). You may use research articles from your reference list. If you cannot find these two types of research on your portfolio topic, you may choose another topic.

  • Locate the following documents in this week’s Learning Resources to access the appropriate templates, which will guide your critique of each article:
    • Critique Template for a Qualitative Study
    • Critique Template for a Quantitative Study
    • Critique Template for a Mixed-Methods Study
  • Consider the fields in the templates as you review the information in each article.
  • Begin to draft a paper in which you analyze the two research approaches as indicated below.
  • Reflect on the overall value of both quantitative and qualitative research. If someone were to say to you, “Qualitative research is not real science,” how would you respond?

    To complete this Assignment:

  • Complete the two critiques using the appropriate templates (see attached files).
  • Write a 2- to 3-page paper that addresses the following:
    • Contrast the types of information that you gained from examining the two different research approaches in the articles that you selected.
    • Describe the general advantages and disadvantages of the two research approaches featured in the articles. Use examples from the articles for support.
    • Formulate a response to the claim that qualitative research is not real science. Highlight the general insights that both quantitative and qualitative studies can provide to researchers. Support your response with references to the Learning Resources and other credible sources.

As you complete this Assignment, remember to:

  • Submit your paper to Grammarly and Turnitin through the Walden Writing Center. Based on the Grammarly and Turnitin reports, revise your paper as necessary.
  • Reminder: The School of Nursing requires that all papers submitted include a title page, introduction, summary, and references. The School of Nursing Sample Paper provided at the Walden Writing Center provides an example of those required elements (available from the Walden University website found in this week’s Learning Resources). All papers submitted must use this formatting.
  • Combine all three parts of this assignment into one Word document including both critique templates and the narrative with your references.  Submit this combined document.

Dissemination of Scholarship Related to the Practicum Project

on: Dissemination of Scholarship Related to the Practicum Project

According to Freemantle and Watt (1994), “effective dissemination encompasses more than just the distribution and diffusion of information. It involves the processes by which target groups become aware of, receive, accept and utilize information.” (p. 133).

Think about your Practicum Project. Which groups would find your project findings, conclusions, and recommendations most valuable? Why? How might your focus differ if you were delivering a presentation at a professional conference versus at your practicum site? What would you emphasize if you were publishing an article in a peer-reviewed journal related to your specialization as opposed to in an online forum with a wider healthcare readership?

In this Discussion, you analyze your Practicum Project in terms of conventional means of scholarly dissemination: presentations, posters, and publication.

To prepare:
  • Reflect on your Practicum Project.
  • Speak with various individuals, especially those in your practicum setting, about presentations or poster sessions they have given and/or publications they have authored. Ask which organizations or journals they would recommend you consider for dissemination based on your Practicum Project topic.
  • Consider the following questions related to dissemination:
    • Who is your target audience? Who would be most interested in your project findings, conclusions, and recommendations? Consider how the needs or dispositions of the intended audience may inform your choice of dissemination method.
    • Which method of dissemination would you choose to convey the findings, conclusions, or recommendations from your Practicum Project? What are the relative benefits and challenges of giving a presentation, sharing your findings through a poster session, and/or publication?
    • Consider various nursing, health care, informatics, and leadership and management organizations and journals. What is one organization or journal you would select for your presentation or publication?
    • What are some potential challenges? Evaluate what you could do to promote the possibility that your presentation, poster, or manuscript would be accepted.
  • Conduct a search of the literature using the Walden Library and other reliable sources. Locate two or more articles published in peer-reviewed journals within the last five years that relate to your Practicum Project. These articles should demonstrate the significance of the topic for nursing leadership and management or nursing informatics. Note: These articles may also help you determine if your project findings are related to an area that will contribute to a more comprehensive literature base.
By Day 3

Post a brief description of your Practicum Project, including the anticipated outcomes and implications for nursing practice in your specialty area. Explain why your results warrant dissemination. Describe your target audience, and explain which method of dissemination you would use as well as the organization or journal you would choose for sharing findings related to your Practicum Project. Support your response with at least two citations from the literature.

Read a selection of your colleagues’ responses.

Cancer, Genetics, and Genomics

Cancer, Genetics, and Genomics
Rachel comes from a family with a history of breast cancer on her mother’s side. Rachel’s mother died of breast cancer when she was very young. Rachel has two sisters, Lisa and Kristin. Rachel has remained close to Lisa, but she no longer has a relationship with Kristin. At a routine checkup, Rachel is told about the availability of genetic testing for identifying a predisposition to breast cancer. Her doctor recommends the test to Rachel given her family history. Rachel has the genetic testing done and finds that she has a mutated breast cancer 1, early onset (BRCA1) gene. Her doctor tells her she is at high risk for developing breast and ovarian cancer. Rachel’s doctor suggests she ask her sisters to be tested also, so they can take the proper preventative measures. Rachel feels comfortable sharing this information with Lisa, but she has not spoken to Kristin in many years. Rachel tells her doctor that she is not in contact with Kristin and will not make an effort to tell her about BRCA1 and genetic testing. Rachel’s doctor feels confident that she can locate Kristin but worries about breaching patient confidentiality if she goes against Rachel’s wishes.

If you were Rachel’s healthcare provider, what would you do? Provide a rationale for your response. Include the pathological processes associated with breast cancer. What role does the BRCA1 gene contribute to managing the patient’s care? Describe and explain the role of the BRCA1 and breast cancer 2, early onset (BRCA2) gene in contribution as a risk factor for breast cancer. Analyze the risk factors for breast cancer and possible interventions to preventive health management for women and men.

Clinical Questions

When you decide to purchase a new car, you first decide what is important to you. If mileage and dependability are the important factors, you will search for data focused more on these factors and less on color options and sound systems.

The same holds true when searching for research evidence to guide your clinical inquiry and professional decisions. Developing a formula for an answerable, researchable question that addresses your need will make the search process much more effective. One such formula is the PICO(T) format.

In this Discussion, you will transform a clinical inquiry into a searchable question in PICO(T) format, so you can search the electronic databases more effectively and efficiently. You will share this PICO(T) question and examine strategies you might use to increase the rigor and effectiveness of a database search on your PICO(T) question.

To Prepare:

  • Review the materials offering guidance on using databases, performing keyword searches, and developing PICO(T) questions provided in the Resources.
  • Review the Resources for guidance and develop a PICO(T) question of interest to you for further study.

Creating a Budget

Instructions
Creating a Budget

In this assignment you will create a spreadsheet and a chart to help a hypothetical couple work out their budget. You will submit a Microsoft Excel spreadsheet following the directions below to the appropriate submission folder by the due date. You will name this file W4P_LastName.xlsx.

Please Note:Information to help you with this project is available in the Week 4 online lectures, your textbook, as well as Microsoft Excel Help (F1). Your professor is also a great resource. If you work ahead, you can post questions about this assignment in the Questions for the Professor area in Course Resources and still submit your assignment on time.

Scenario

Tom and Sally were trying to get a handle on their budget. They heard that MS Excel could help them with that. Tom is going to school and has cut back on his hours. Sally has finished her associate’s degree and is working full time. They want to look at the last three months of their income and expenses to see where they can make changes.

Part I – The Spreadsheet

First, in an Excel Spreadsheet, create a budget to show Tom and Sally’s income, expenses, and money leftover each month (Net). Create formulas in your spreadsheet for your calculations using the following data:

Tom brought home $1,000 a month for January, February, and March.
Sally brought home $1,900, $2,000, and $1,975 respectively for those three months.
They paid $1,000 for rent and utilities each month.
They paid $88 for insurance each month.
They paid $60 for cell phones and $60 for Internet/TV each month.
They spent $600, $750 (due to a big birthday bash), and $500 on food respectively.
Their car payment and gas came to $225, $250, and $300 respectively.
Entertainment and gifts were $75, $100, and $45 respectively.
They paid a little extra on their credit card $150, $125, and $100 respectively.
They allocated $200 each month for personal grooming and health care.
They put $100 each month into savings.
Miscellaneous Expenses came to $200, $100, and $400 respectively.
A sample budget layout is included below as an example of one way to lay out a budget.

Example Budget Layout

Part II – The Chart

Once you have worked out the budget spreadsheet, use the tabs at the bottom of the page to open another sheet. Here you will create a chart similar to the example below using the tools you learned about in the lectures and the textbook. On the new sheet:

Create a column chart to visually represent their monthly expenses. If you have a challenge getting the chart on this sheet, research how to move your chart using your reading or Help (F1). You might also try right clicking on the chart.
Add a descriptive title to the chart.
Example Line Chart

Part III – Changing Values

Copy everything from Sheet 1 (the budget calculations) and paste it into a new worksheet.
Change values for Sally’s March income to $200 more. Let’s say that they didn’t use as much heat in March and their utilities were $75 less (you could use a formula to figure out $75 less but you don’t have to). See how the totals change automatically if you used formulas correctly. If the values did not change, check your formulas and try them again. Seek help if you can’t figure this out.
Rename each of your three spreadsheets with descriptive names.
By the assigned due date, submit this budget to the appropriate submission folder. In the message box, share how the process of creating this went for you, including any challenges, successes, or insights.
