abnormal findings in patients

For this assignment, you will analyze an Episodic note case study that describes abnormal findings in patients seen in a clinical setting. You will consider what history should be collected from the patients, as well as which physical exams and diagnostic tests should be conducted. You will also formulate a differential diagnosis with several possible conditions. Please remember to pretend that this is an actual patient and gives as much detail as possible!

Patients are frequently uncomfortable discussing with healthcare professional’s issues that involve the genitalia and rectum; however, gathering an adequate history and properly conducting a physical exam are vital. Examining case studies of genital and rectal abnormalities can help prepare advanced practice nurses to accurately assess patients with problems in these areas.

In this Lab Assignment, you will analyze an Episodic note case study that describes abnormal findings in patients seen in a clinical setting. You will consider what history should be collected from the patients, as well as which physical exams and diagnostic tests should be conducted. You will also formulate a differential diagnosis with several possible conditions.

To Prepare

  • Review the Episodic note case study your instructor provides you for this week’s Assignment. Please see the “Course Announcements” section of the classroom for your Episodic note case study.
  • Based on the Episodic note case study:
    • Review this week’s Learning Resources, and consider the insights they provide about the case study. Refer to Chapter 3 of the Sullivan resource to guide you as you complete your Lab Assignment.
    • Search the Walden library or the Internet for evidence-based resources to support your answers to the questions provided.
    • Consider what history would be necessary to collect from the patient in the case study.
    • Consider what physical exams and diagnostic tests would be appropriate to gather more information about the patient’s condition. How would the results be used to make a diagnosis?
    • Identify at least five possible conditions that may be considered in a differential diagnosis for the patient.

The Lab Assignment

Using evidence-based resources from your search, answer the following questions and support your answers using current evidence from the literature.

  • Analyze the subjective portion of the note. List additional information that should be included in the documentation.
  • Analyze the objective portion of the note. List additional information that should be included in the documentation.
  • Is the assessment supported by the subjective and objective information? Why or why not?
  • Would diagnostics be appropriate for this case, and how would the results be used to make a diagnosis?
  • Would you reject/accept the current diagnosis? Why or why not? Identify three possible conditions that may be considered as a differential diagnosis for this patient. Explain your reasoning using at least three different references from current evidence-based literature.

Assessment Instructions

Preparation

As you prepare for this assessment, complete the following:

  1. Before you begin, examine your organization’s history of safety in a specific area and how your organization addresses patient safety issues. If possible, consult with a key stakeholder in the organization (such as an administrator) to better understand specific patient-safety concerns and how the organization is working to resolve the concerns. This person should also be able to discuss some of the organizational barriers impacting the patient safety issue.
  2. Next, look at the basic concepts, principles, and practices that contribute to organizational quality improvement and patient safety. Review the literature for best practices and how technology might be used to improve the issue.
  3. Finally, be sure to consider the legal and ethical implications associated with the safety issue, as well as possible organizational barriers to change.

Directions

As you construct this assessment, address each point as completely as possible:

  • Describe a patient-safety issue within your organization, comparing the way your organization addresses the issue with the concepts, principles, and practices that contribute to quality improvement and patient safety.
  • Analyze the legal and ethical consequences of not addressing the issue.
  • Recommend evidence-based interventions, including technology, to address the patient-safety issue.
  • Describe strategies to overcome specific organizational barriers to change, based on your knowledge of the organization.

Additional Requirements

Pain

Discussion: Pain

The neurological system affects all parts and functions of the body through nerve stimulation. Nerves also control the sensation and perception of pain. While pain can be described in a variety of ways, it is essentially labeled according to its duration and source. As an advanced practice nurse evaluating a patient, you need to consider the following questions: Does the pain quickly come and go, or is it persistent and ongoing? Does the pain arise at the source of injury or in another location? In this Discussion, you compare three common types of pain—acute, chronic, and referred.

To Prepare

· Review this week’s media presentation on the neurological system, as well as Chapter 14 in the Huether andMcCance text.

· Identify the pathophysiology of acute, chronic, and referred pain. Consider the similarities and differences between these three types of pain.

· Select two of the following patient factors: genetics, gender, ethnicity, age, or behavior. Reflect on how the factors you selected might impact the pathophysiology, diagnosis, and prescription of treatment for acute, chronic, and referred pain.

Post a description of the pathophysiology of acute, chronic, and referred pain, including similarities and differences between them. Then, explain how the factors you selected might impact the pathophysiology, diagnosis, and prescription of treatment for acute, chronic, and referred pain.

Required Readings

Huether, S. E., & McCance, K. L. (2017). Understanding pathophysiology (6th ed.). St. Louis, MO: Mosby.

  • Chapter 14, “Pain,      Temperature, Sleep, and Sensory Function”

This chapter covers the role of pain, sleep, stress, and the senses on body functions. It also explores alterations involving pain, sleep, stress, and the senses.

Instructor Requirements

As advanced practice nurses, we are scholars, nurse researchers and scientists. As such, please use Peer-Reviewed scholarly articles and websites designed for health professionals (not designed for patients) for your references. Students should be using the original citation in Up to Date and go to that literature as a reference. The following are examples (not all inclusive) of resources/websites deemed inadmissible for scholarly reference:

  1. Up      to Date (must use original articles from Up to Date as a resource)
  2. Wikipedia
  3. Cdc.gov-      non healthcare professionals section
  4. Webmd.com
  5. Mayoclinic.com

Simulation in Healthcare

Simulation in Healthcare

The discussion assignment provides a forum for discussing relevant topics for this week on the basis of the course competencies covered.

By the due date assigned, post a response to one of the two discussion topics below, using information from the lectures, reading assignments, library resources, and Internet resources. All responses should be posted to the Discussion Area.

To support your work, use your course and textbook readings and also use the South University Online Library. As in all assignments, cite your sources in your work and provide references for the citations in APA format.

Tasks:

Topic 1

Simulation technology has been transformative in nursing education. Through the use of simulation, nurses can repeatedly practice skills and gain clinical confidence in a risk-free environment. Simulation, however, is not just for health care professionals. How can nurses leverage simulation technology for patient and family education? Share your experiences and ideas for leveraging simulation technology for patient education.

Citations should conform to APA guidelines. You may use this APA Citation Helper as a convenient reference for properly citing resources or connect to the APA style website through the APA icon web address below.

Practicum Experience

Assignment: Practicum Experience

As your practicum proceeds, consider the topics addressed this week. Conduct research to further your understanding of how issues related to health information systems and other resources, such as financials, staffing, and materials, could influence the implementation of your proposed change.

In addition, you reflect on the effectiveness of your practicum experiences for fostering your professional development. What challenges are you encountering, or would you anticipate at this point? If necessary, what can be done to improve the situation?

To prepare:

  • Reflect on the learning objectives and      activities you developed for your practicum experience during Week 1 (See      attached file). Also, consider the leadership competencies introduced in      Weeks 2 and 3.
  • What new skills or insights have you      gained thus far in your practicum experience? How do you think this is      impacting your leadership development and the planning of your proposed      change? What adjustments might need to be made to your leadership presence      within the practicum environment—or your plan for change? What additional      supports would you find most beneficial right now?

To complete: By tomorrow 4/10/19 6mpm, write a minimum of 550 words essay in APA format with at least 3 scholarly references. Include the level one headers as numbered below:

1) What new skills or insights have you gained through your practicum experiences?

2) How is this impacting your leadership development and planning related to your proposed change?

3) What additional supports would be beneficial?
