Using EBPH to Define and Quantify the Extent of the Public Health Problem
Note: Your response and comments to this assignment will be laying the foundation for work that you will later complete as part of your final project.

A clear statement of the issue provides a concrete basis for a priority setting process that is objective. This then leads to better program planning, intervention, and evaluation. A fully articulated issue statement includes a complete description of the problem, potential solutions, data sources, and health-related outcomes.

In order to provide a complete description of the problem, the problem must first be quantified. In quantifying the health issue or disease, the application of the appropriate epidemiologic tools is necessary to obtain the answers to certain questions while conducting the community assessment. The community assessment should include the health condition or risk factor being considered, the population affected, the size and scope of the problem, prevention opportunities, and potential stakeholders. This task requires basic epidemiologic skills to obtain additional information about the frequency of the health condition or risk factor in an affected population.

Going back to the public health problem, issue, situation, or concern that you identified for W1: Assignment 3, develop a concise statement of your problem. Your statement must be clear and well-articulated including a description of the problem, potential solutions, data sources, and health-related outcomes. The statement of your problem will serve as the basis for your research purpose or objective, which will be very critical in the application of the evidence-based process which in turn leads to better program planning, program intervention, and program evaluation.

Using the strategies and concepts taught for searching, evaluating, and retrieving EBPH resources and literature, identify five (5) peer-reviewed scientific articles that have information that can be used to quantify the extent of your specific health problem. Ensure that none of these articles are more than 5 years old.
Analyze and evaluate three of the five articles with the intent to quantify the extent of your public health problem, issue, situation, or concern.
Use the epidemiology training and skills from the course to assess the extent of your problem, issue, situation, or concern by identifying and evaluating the risk factors being considered, the population affected, the size and scope of the problem, prevention opportunities, and potential stakeholders from the three peer-reviewed articles.
As part of quantifying the public health condition, obtain the disease rates of the health condition or risk factor in an affected population and analyze and assess if any patterns of the disease exist within subgroups of the defined population.
Utilize assigned and suggested weekly readings, the South University online library, the Internet readings, and references to search, evaluate, and retrieve relevant EBPH literature to support your responses.

As noted in the Learning Resources, systems theory provides a meaningful and beneficial means of examining challenges in health care organizations. To do this effectively, however, it is essential to assess all system components, as some may be relatively healthy while others are problematic.

 

For this Assignment, you apply systems theory to the examination of a problem in a department or a unit within a health care organization. (Note: You may use the same problem you identified for the Discussion as long as it meets the criteria for this assignment.)

 

To prepare:

 

  • Review the Meyer article, “Nursing Services Delivery Theory: An Open System Approach,” in this week’s Learning Resources. Focus especially on the information presented in Table 1 (p. 2831) and Figure 2 (p. 2833).
  • Reflect on your organization or one with which you are familiar ( Refer to the first discussion you did for me, My organization is a hospital University of Maryland Medical center) . Within a particular department or unit in this organization, identify a problem the staff is encountering.
  • Using Table 1 in the Meyer article as a guide, analyze the department or unit, identifying inputs, throughput, output, cycles of events, and negative feedback. Consider whether the problem you have selected relates to input, throughput, output, cycles of events, and/or negative feedback.
  • Think about how you could address the problem: Consider what a desired outcome would be, then formulate related goals and objectives, and translate those goals into policies and procedures.
  • Research professional standards that are pertinent to your identified problem.
  • Reflect on the organization’s mission statement and values. In addition, consider how addressing this problem would uphold the mission and values, while improving the organizational culture and climate. (Depending on the organization you have selected, you may have explored these in the Week 1 Discussion.)

Adolescents

Youth suicide is the third leading cause of death for persons between 15-24 years of age, and almost 4,600 youth deaths each year are the result of suicide for a person 10-24 years of age (Smischney, Chrisler, & Villarruel, 2014). Learning of these numbers is very discouraging considering that suicide can be prevented by recognition and implication of interventions. Adolescents may present to family, friends, or teacher’s signs of suicidal behavior such as talk of suicide, threat of suicide, or risky behavior. Sometimes the adolescent may not display warning signs before committing suicide. It is important to identify risk factors that can lead to suicide. Risk factors that contribute to suicidal ideation are biological, environmental, and psychological factors (Smischney et al., 2014).

Biological risk factors include gender, ethnicity, and sexual orientation. The male gender is 4 times greater to attempt suicide that results in death, whereas female adolescents experience higher rates of depression. Native American or Alaska Natives that are between the ages of 15-24 are at a 2.4 percent higher rate than the national average. Suicidal ideation is higher amongst gay and bisexual male adolescents than heterosexual male adolescents. This may due to the adolescent’s parents or friends lack of approval or support (Smischney et al., 2014).

Environmental risk factors that contribute to suicide include family stress and conflict such as divorce, death of a loved one, academic failure, and abuse. During adolescence, peer relationships greatly contribute to suicide. Adolescents who suffer from poor social skills, low self-esteem, and lack support from their peers are at greater risk for suicidal ideation (Smischney et al., 2014).

Psychological risk factors contributing to adolescent suicide include mental health problems, psychiatric disorders, poor coping skills, and substance abuse. Mental health disorders include anxiety, depression, post-traumatic stress disorder, and schizophrenia. Alcohol is often experienced with by adolescents. Female adolescents are 3 times more likely to attempt suicide and male adolescents are 17 times more likely to attempt suicide when alcohol is involved (Smischney et al., 2014).

Primary, secondary, and tertiary heath prevention measures can be taken to prevent suicide. Primary prevention can be implemented by addressing the topic of suicide with adolescents, identifying risk factors of suicide, and talking about ways to avoid risk factors that can lead to suicide. Secondary prevention can be done by addressing risk factors that the adolescent is experiencing and implementing healthy and effective interventions. This will help to reduce the chance of the adolescent following through with the act of suicide. Tertiary prevention should include providing support and resources to the adolescent, as well ensuring safety. Measures should be taken to prevent the adolescent from attempting and succeeding at suicide.

The Suicide Prevention Resource Center is a resource that provides contact information and suicide prevention plans specific for each state. This information can be accessed through the website http://www.sprc.org/states. Adolescents can also contact the National Suicide Prevention Lifeline 24/7 by calling 1-800-273-8255, or going online to https://suicidepreventionlifeline.org/. Both of these resources offer support to those who are experiencing a suicidal crisis. As a nurse if you suspect a depressed adolescent is in immediate danger of harming themselves, immediate intervention should be implemented such as ensuring the safety of the adolescent. If the nurse is physically present at the adolescent’s side, taking the adolescent to a safe environment and informing a physician is important to prevent harm or injury. If the nurse is talking with the adolescent over the phone and the adolescent is posing immediate danger to themselves, proper authorities should be notified and full detail of the adolescent’s location and situation should be provided.
