Nursing Intervention

Review the Topic Materials and the work completed in NRS-433V to   formulate a PICOT statement for your capstone project.

A PICOT starts with a designated patient population in a particular   clinical area and identifies clinical problems or issues that arise   from clinical care. The intervention should be an independent,   specified nursing change intervention. The intervention cannot require   a provider prescription. Include a comparison to a patient population   not currently receiving the intervention, and specify the time frame needed to implement the change process.

Formulate a PICOT statement using the PICOT format provided in the   assigned readings. The PICOT statement will provide a framework for   your capstone project.

In a paper of 500-750 words, clearly identify the clinical problem   and how it can result in a positive patient outcome.

Physical Examination

Physical examination is the evaluation of anatomic findings by using observation, percussion, palpation, and auscultation to obtain information about the patient. Many people who visit healthcare providers follow their instructions but wonder what they are doing or what they are looking for. During a physical examination, a healthcare practitioner is gathering cues to be able to diagnose. When a physical examination is thoughtfully integrated with the information that they provide, history, and path physiology, they should yield at least 20% data that is necessary for diagnosis and management of the patient (Sawyer, 2012). An examination of a 12-year-old child to find the likely cause of the symptoms portrayed would assist in diagnosing the child.

You are admitting a 12-year-old child to your unit. The mother states that the child has a history of unexplained blackout episodes, headaches, sleep disturbances, and is presently exhibiting tremors. What is the most likely cause of these symptoms? What actions would you take during the interview process? Explain.

I have chosen the 12 year old patient for this discussion.

When conducting a physical assessment on children it is imperative to start the collaboration relationship process between the patient and their family members and myself, as the nurse with effective communication strategies. I would first introduce myself to the patient and mother of the patient and explain to the both of them the purpose of the assessment being performed and how the information that they provide will be utilize appropriately. One important aspect to explain to both of them is that the information that they provide to me is protected by HIPAA.

I would then use open-ended questions (i.e. what brings you in today) to direct the interview to gain the patient’s history from either the child or from the mother. If more information is needed then I would utilize closed-ended questions or direct statements (i.e. how long has these symptoms been going on) to clarify any additional information.

Next, I would take the patient’s vitals and a complete head-to-toe assessment with an emphasis on the neurological system exam, all while explaining to the patient and the mother the reason for me having to do this. By me explaining step-by-step what I am doing will help me to better build a rapport with them as well as giving the patient and the mother a sense of being aware of what is going on, so that if they have any questions I will be able to answer these for them.

It would be pretty apparent that with the symptoms that the patient is experiencing may be due to some type of neurological condition. Patient would then need to be referred to a neurologist for further testing to confirm a proper diagnosis.

Considering all aspects of the patient medical history, including the history of close family members, is important to ensure the complete clinical picture is evident. Asking if family members have a history of depression is an important point we must raise during a history assessment of our first patient. Would you agree?

evidence-based practice project.

After discussion with your mentor, name one financial aspect, one quality aspect, and one clinical aspect that need to be taken into account in developing the evidence-based practice project. Explain how your proposal will, directly and indirectly, impact each of the aspects.

Speaking with the mentor, the financial aspect of the project would be hours spent trying to get nurses educated on the proposed guidelines for late preterm infants. Hours will be needed as well to get the basic checklist implemented into the computer in this particular area, which requires going to different committees and getting approvals needed. This takes more than one nurse or person to get this done. This requires more hours of pay, and that would be the financial aspect of the proposed project.

Now, the clinical aspect that has to be taken into account with developing the project would be how the nurses will accept the project. As lifelong learners, staff nurses continually encounter new knowledge of relevance to their patients. The conscientious nurse remains alert to clinical problems and encourages investigation into ways to improve patient outcomes(Di Lenoardi, 2014). The nurses need to be given research and data that proves this could help readmission of late preterm infants. If given this data, they would be involved in the process of implementing as well,

In 2001, the Institute of Medicine (IOM) recommended evidenced based practice to improve healthcare outcomes(Stephens, 2013). This project is to improve the quality outcome of late preterm infants and prevent their readmission to hospital.

All the aspects brought together will make a good impact on the project the author believes with the research and data given to the nurses, implementation and then follow up of results will show this is a great solution to help alleviate many readmissions nationwide as well. If this hospital shows that the guidelines are helping there, then they can show other colleagues in the nation that this helps and also will cut costs by preventing readmissions.

Adult Gerontology Primary Care Nurse Practitioners (AGPCNP) are trained to care for individuals age thirteen years of age and older (Walden University, n.d.). AGPCNP’s are involved in the direct care of patients. AGPCNP’s can evaluate, make a diagnosis, and treat a range of health concerns (Walden University, n.d.).

Reason for Choice

The influence that obtaining an advanced degree provides an individual is an educational background for explaining and providing standards of practice (Laureate Education, 2018). Being able to provide direct patient care and doing so with standards of practice is the path that inspires me the most. I want to be able to care for patients to the best of my ability with the knowledge I have gained from experience and education. I chose AGPCNP related to I to want to be able to care for the elderly population, as this is the field I am most passionate about. I did have a hard time choosing acute or primary and finally decided to go with primary as I hope to open my own clinic one day.

Organization

One organization is Gerontological Advanced Practice Nurses Association (GAPNA). GAPNA is an organization that advocates for the quality care for the elderly, promote professional growth for healthcare professionals, offers continuing gerontological education, and improves communication and professional teamwork among healthcare providers (Gerontological Advanced Practice Nurses Association, 2020). This organization will keep me updated on several issues needed for my professional career to helping improve patient care.

-Phenomenological

4-Phenomenological research and grounded theory are common methods of conducting qualitative research.

Similarities:

·          Both collect data from their subjects’ perceptions and perspectives

·          Both do their best to make sure their findings are not influenced by preconceived notions or judgments

·          Both explore their participants’ personal experiences

·          Both seek understanding the meaning of events and experiences on peoples’ lives, (Driessnack, 2007)

Differences:

·          Phenomenological research focuses on understanding the impact an experience has on a person’s life. The only sources of information are the people experiencing the event (phenomena). This type of research is often limited to interviews only, (Driessnack, 2007)

·          Grounded theory research includes a variety of data sources, including diaries, observations, images, and past research. They compare the data they collected with older data and look for differences and contradictions, as well as social patterns.

5-“The purpose of phenomenological research is to provide a thorough description of a lived experience. Some researchers will summarize their findings with a written summary that combines the findings into a thorough description or an exemplar of the experience” (Grove 70).

“Grounded theory research is an inductive technique that emerged from the discipline of sociology. The term grounded means the theory developed from the research has its roots in the data from which it was derived. Most scholars base the grounded theory methodology on symbolic interaction theory. George Herbert Mead (Mead, 1934), a social psychologist, developed symbolic interaction theory, which involves exploring how people define reality and how their beliefs are related to their actions. Reality is created by attaching meanings to situations. Meaning is expressed in such symbols as words, religious objects, patterns of behavior, and clothing. These symbolic meanings are the basis for actions and interactions. However, symbolic meanings are different for each individual, and we cannot completely know the symbolic meanings for another individual” (Grove 70-71).

“Grounded theory has been used most frequently to study areas in which little previous research has been conducted and to gain a new viewpoint in familiar areas of research. Through their interviews to understand the perspectives of persons who were dying, Glaser and Strauss (1967) developed grounded theory research as a method and published a book describing it as a qualitative method. Nurses were attracted to the method because of its applicability to the life experiences of persons with health problems and its potential for developing explanations of human behavior (Wuerst, 2012)” (Grove 71).

“Ethnographic research was developed by anthropologists as a method to study cultures through immersion in the culture over time. The word ethnography means “portrait of a people.” Anthropologists study a people’s origins, past ways of living, and ways of surviving through time—in other words, their culture. Culture is the focus of ethnography. Early ethnography researchers studied primitive, foreign, or remote cultures (Savage, 2006). Such studies enabled the researcher who spent a year or longer in another culture to acquire new perspectives about a specific people, including their ways of living, believing, and adapting to changing environmental circumstances. This reflects the emic approach, one of studying behaviors from within the culture that recognizes the uniqueness of the individual (Ponterotto, 2005). The emic view from inside the culture is the typical goal of ethnography” (Grove 74).

I feel as though phenomenological research and grounded research have the most similarities due to the fact they both want to get to the root of the research by analyzing data from the core. Both types of research analyze both, people and experiences. I prefer grounded research because I like to investigate and analyze current and new research to understand it and create a different viewpoint, and maybe even learn about some of my own discoveries related to that research.

6-I use the online lecture notes to my advantage. They are very helpful for discussion questions. I do not have a hard copy of either the Burns and Gray textbook or the APA manual. I use online information about 99% of the time. I go to owlpurdue.com when I have questions about APA format/quotes. I feel as though it is quicker to have access to online materials whether your are at home on the computer or on your phone. Technology has took off in the last few years.

evaluation of seizure activity

An older client was recently discharged from the hospital for evaluation of seizure activity. His history reveals that he has late-stage Alzheimer’s disease, Parkinson’s disease, hypertension, and type II diabetes mellitus, which is controlled by diet. He lives at home, where his wife and daughter take care of him. His discharge medications include phenytoin (Dilantin), 100 mg BID; hydrochlorothiazide (HydroDIURIL), 50 mg QD; levodopa (Sinemet), 25/100 TID; and haloperidol (Haldol), 1 mg before bed. The client has been referred for home care nursing follow-up.

Questions:
