Assignment Guidelines
Create a SOAP note of a patient who presents with one of the following goals (your choice):
• I want to lose weight. How can you help me?
• I want to quit smoking. How can you help me?
• I want to try to lower my blood pressure naturally. How can you help me?
• I want to lower my cholesterol. How can you help me?
• I want to reduce my stress. How can you help me?
Be sure to include all the components of a SOAP note illustrated in the instructional materials. Please ensure that both pharmacological and non-pharmacological recommendations are included in the treatment plan. Don’t forget to follow up.

By the end of this assignment, you will be able to:
• Identify routine adult health screenings and immunizations. (CO2)
• Identify appropriate interventions needed to help patients maintain healthier lifestyles. (CO3, CO8)
• Use SOAP notes to record patient observations, assessments, and plans. (CO1, CO2, CO8)
