Disorder Analysis Project The purpose/objective of this assignment is the development of a scholarly case study presentation. The student will apply mental health clinical assessment and scholarly literature review to the development of an evidence-based care plan for a client of the student pairs choosing. Elements of the assignment include: 1. Detailed mental status examination 2. Development of a relevant clinical PICOT question 3. Completion of a literature review to answer the PICOT question 4. Development of an evidence-based nursing care plan for the specific client described. 5. The case study will be presented as a scholarly presentation to your professional peers supported by a professional level PowerPoint presentation. Assignment Requirements: This assignment builds in stages. The student pair will choose a client from their clinical setting with a diagnosis of a mood disorder a psychotic disorder PTSD or a co-occurring substance abuse disorder (in addition to any of the prior diagnoses). Other diagnoses may be considered with approval of the instructor. The first step is the completion of a mental status examination on the chosen client and development of a problem list. Next the students develop a research question based on the clients clinical needs using the most important of your three nursing diagnoses. (The focus of the literature search and the care plan will be to answer this question.) The students will then complete a literature search for a minimum of five scholarly research and evidence-based practice articles. Each article which will be analyzed using the worksheets provided and turned as a research bundle of the completed worksheets. The final version of the disorder analysis presentation will be completed according to the detailed rubric below and presented to your clinical group on the last clinical day of the semester. Sources of information: Direct interview of client personal observation of milieu consultation with treating staff (psychiatrist and other multidisciplinary clinical team members) other academic psychiatric literature (e.g. psychiatric textbooks) research studies from peer reviewed mental health/nursing journals and evidence based practice articles from peer reviewed nursing journals are required. Secondary sources are not acceptable unless approved by the instructor. Presentation: The presentation should be no more than 20 minutes long supported by a PowerPoint presentation. Assignment weight/points: Combined pre-writing assignments and final presentation is worth 10 % of course grade. DAP Pre-writing Assignments a. Mental Status Examination (MSE): See rubric for Mental Status Examination. The MSE provides the clinical database for your presentation. Graded by the clinical instructor. b. Research Question: Based on your assessment develop a question in modified PICOT format. Submit to lecture instructor. Research Question should include: Client Problem: What is the significant client problem to which evidence-based care will be applied? Research direction: What area of evidence-based practice do you intend to explore? Intervention: What do you hope to learn in the literature review to support your care plan interventions? (Specific therapies medications education etc.) Outcome: What measurable client outcomes do you seek? (Less symptoms no symptoms sobriety medication adherence etc.) Time: What is the time frame? The time frame should be realistic in the care setting. c. Research Bundle The purpose of the Research Bundle is to gather and critically evaluate reference material related to your PICOT question. Your Halter text is not an acceptable source. Submit to lecture instructor a completed worksheet for each study. Note that there are two forms. One is for research studies. The other is for all other types of scholarly sources. Evaluate four sources possibly including: A) Theoretical books and/or articles to understand the disorder. B) Research studies (qualitative and/or quantitative) related to the dynamics of the disorder medication management and/or clinical management. C) Scholarly nursing research studies consensus papers and other scholarly sources regarding evidence-based practices related to the clients care planning and/or medication needs. Disorder Analysis Paper Research Bundle Worksheets Research Study: (citation in APA format) Why was the study done? (What was the problem under investigation and the purpose (research question) of the study?) How closely does the focus of the study relate to your area of interest? Who was studied? Describe the population included in the study sample. Include the number of participants. What concerns do you have about generalizing from this population to your client? How was the study done? A. Identify the research methodology: 1. Quantitative: e.g. descriptive correlational cohort randomized controlled quasi-experimental 2. Qualitative: e.g. ethnography grounded theory phenomenology 3. Mixed method: a bit of both (e.g. descriptive survey plus phenomenology) B. What was the intervention? Discuss how the study was carried out? What variables were measured and/or compared? How were the variables measured? What was the outcome? Briefly summarize the research results. How you will apply this data to your DAP care plan? Summary of strengths/limitations/relevance of the study and its findings for use in your DAP? Scholarly Review or Theoretical Article: (citation in APA format) Description of source? (Integrative review systematic review meta-analysis Meta-synthesis Evidence-based clinical practice guideline? See Davies and Logan p. 31) Significant data/evidence based practices? Strengths/limitations of the data/evidence presented? For example did the author present sufficient scholarly support for practice implications or other conclusions? Relevance of the clinical/theoretical material presented to your client/ PICOT question? What concerns do you have about generalizing from this information to your clients care planning needs? How can you use the results? Briefly describe the implications (i.e. how this study can /cannot be applied) to your problem of interest and your DAP care plan. Disorder Analysis Project Presentation The final presentation will be evaluated for thesis audience following directions support and professional writing style/grammar/usage/spelling professional presentation and APA references. Note that this assignment description is quite detailed. The time limit of 20 minutes will be strictly adhered to. Therefore all information must be summarized and bulleted effectively to achieve a successful presentation. See below for suggested slide layout. Slides should include citations for referenced data and a reference list in APA format. Disorder Analysis Project PowerPoint Presentation Guidelines Slide #1: The title of your project Slide #2: Research question with discussion of the significance of the question. Slide #3: Introduce your client by using the demographic data and Axes I-V on the first page of the Mental Status Examination (MSE) form. Slide #4: Present only the pertinent/positive findings of the MSE. There is no need to mention those sections of the MSE which reflect no problem. Slide #5: Medication table. The client may be taking many medications but you should focus only on the psychotropic medications and why this particular client is taking them. Cite a minimum of two scholarly references in addition to Halter on this slide. Sample Medication Table Medication Classification Action Dosage and Route Side Effects Rationale for use in this client Aricept (Donepezil hydrochloride) Cholinesterase inhibitor Inhibits the breakdown of endogenously released acetylcholine. 5 mg po every morning Monitor for gastrointestinal bleeding. Mild drowsiness headache metallic aftertaste allergic dermatitis Used for the treatment of mild to moderate dementia of the Alzheimers type. This client currently exhibits moderate memory loss. Zyprexa (Olanzapine) Atypical Antipsychotic Blocks serotonin receptors and D2 receptors. 10 mg/day po every evening Somnolence agitation nervousness insomnia dizziness weight gain constipation dry mouth increased appetite Used for the treatment of psychotic symptoms of dementia. This client exhibits auditory hallucinations and paranoid delusions. Slide #6: The three most important nursing diagnoses for the subject of your presentation and which of these diagnoses led to the formulation of your PICOT question. Briefly discuss rationale for this nursing diagnosis based client problem and literature findings. Slides #7 +: The nursing interventions for the chosen nursing diagnosis for this client and how these interventions are supported by the literature. Based on your research develop three interventions with rationales that are: a) Client specific b) Behaviorally measurable with timeframes c) Realistic and appropriate to the clinical setting. d) Based on the students literature search (with citations). Evaluation: If you were able to implement the intervention discuss the actual client response. If not implemented discuss the hoped-for result and how it would be behaviorally evaluated. As in all care plans evaluation criteria must be client specific setting specific measureable and have a clear timeframe. When a source is cited please make sure you note it on the slide in APA format just as you would in a paper. There may be research articles which dont support your proposed nursing interventions. However the preponderance of scholarly citations should support the interventions you would use with this client. This section should be heavily referenced but your Halter textbook is not an acceptable source for this section. Cite a minimum of two primary research studies in your discussion. In addition scholarly books and theoretical articles from peer-reviewed journals are appropriate. Slide: #? Conclusion: This slide should briefly summarize your presentation and answer your research question. Briefly identify practice implications and needs for further study. Slide: #? References: All sources cited in the presentation are listed at the end of the Power Point presentation in APA format and should be in 20-point font in order for the audience to be able to see them. Students are expected to explain the approaches and interventions available to support individuals with specific needs.Furthermore students must describe the potential impact of challenging behaviour on health and social care organizations and analyse strategies for working with challenging behaviours.Case Study 1Baby Peter was failed by all agenciesPeter Connelly (also known as Baby P) was a 17-month-old English boy who died in London after suffering more than fifty injuries over an eight-month period during which he was repeatedly seen by Haringey Childrens services and NHS health professionals.Peter Connelly was born to Tracey Connelly on 1 March 2006. In November Connellys new boyfriend Steven Barker moved in with her. In December a GP noticed bruises on Peters face and chest. His mother was arrested and Peter was put into the care of a family friend but returned home to his mothers care in January 2007. Over the next few months Peter was admitted to hospital on two occasions suffering from injuries including bruising scratches and swelling on the side of the head. Connelly was arrested again in May 2007.In June 2007 a social worker observed marks on Peter and informed the police. A medical examination concluded that the bruising was due to abuse. On 4 June the baby was placed with a friend for safeguarding. Over a month later on 25 July Haringey Councils Children & Young Peoples Service obtained legal advice which indicated that the threshold for initiating Care Proceedingswas not met.On 1 August 2007 Baby Peter was seen at St. Anns Hospital in North London by locum paediatrician Dr. Sabah Al-Zayyat. Serious injuries including a broken back and broken ribs very likely went undetected. The next day an ambulance was called and Peter was found in his cot blue and clad only in a nappy. After attempts at resuscitation he was taken to North Middlesex hospital with his mother but was pronounced dead at 12:20 pm. A post-mortem revealed he had swallowed a tooth after being punched. Other injuries included a broken back broken ribs mutilated fingertips and fingernails missing. The police immediately began a murder investigation and Baby Ps mother was arrested. So too were Barker his brother Jason Owen and his 15-year old girlfriend who had fled to and were hiding in a campsite in Epping Forest.Baby Ps real first name was revealed as Peter on the conclusion of a subsequent trial of Peters mothers boyfriend on a charge of raping a two-year-old.The case caused shock and concern among the public and in Parliament partly because of the magnitude of Peters injuries and partly because Peter had lived in the London Borough of Haringey North London under the same child care authorities that had already failed ten years earlier.Peters mother Tracey Connelly her boyfriend Steven Barker and Jason Owen (later revealed to be the brother of Barker) were all convicted of causing or allowing the death of a child the mother having pleaded guilty to the charge. The child protection services of Haringey and other agencies were widely criticised. Following the conviction three inquiries and a nationwide review of social service care were launched and the Head of Childrens Services at Haringey was removed by direction of the government minister.A report by Graham Badman suggested that Baby Peters horrifying death was down to theincompetence of almost every member of staff who came into contact with him.The report stated that the practice of the majority both individually and collectively was incompetent.Their approach was completely inadequate and did not meet the challenge of the case it argued.Source: http://www.bbc.co.uk/news/educationCase study Two:The Bournewood CaseRights for vulnerable people in the care systemMr. and Mrs. E live in a picturesque cottage in a quiet Surrey Village. Inside the house is buzzing with activity: three Old English sheepdogs rescued from a home for abandoned animals roam about the kitchen and patio. Photos of family outings cover the walls. Footsteps on the stairs signal the entrance of HL the autistic man for whom Mr. and Mrs. E are carers. He pauses for a silent greeting before making his way swiftly to the fridge. He knows exactly where we keep his favourite juice laughs Mrs. E.Mr. and Mrs. E are remarkable people whose struggle for HLs human rights has changed the way vulnerable people are treated under British law.HL came to live with Mr. and Mrs. E in 1994 under a resettlement scheme from Bournewood hospital where he had lived for 32 years. With their children grown up the couple had decided to open their home to someone who needed it. Looking after HL was no easy task: he cannot talk and needs help with basic tasks like washing and dressing himself. Mrs. E says: Its fair to say that it was a challenge but it was rewarding to see how much HL benefited from living in a family setting. At first he was very institutionalised but he gradually became more confident and progressed beyond all expectations.A requirement of his placement was that HL would attend a day centre once a week to which he travelled by the centres transport. On July 22nd 1997 three years after he had come to live with Mr. and Mrs. E it was not the usual driver who collected from their home. Rather than taking him straight to the day centre as normal the driver took a different route collecting others on the way. HL became increasingly agitated. The next thing Mr. and Mrs. E knew was that HL had been taken back into Bournewood hospital and detained there. He had been admitted informally using a clause in the Mental Health Act 1983 under which the hospital simply had to argue that it was in his best interests and as HL cannot speak he was unable to object. Mr. and Mrs. E were not allowed to visit him apparently in case he wanted to leave with them. They sent us a letter thanking us for agreeing not to visit says Mrs. E. We hadnt agreed anything they had decided without any consultation. When Mr. and Mrs. E realised that HL was not going to be allowed home they engaged a solicitor on his behalf and took a case for unlawful detention to the High Court which ruled against him. The Appeal Court overturned the decision in October 1997 and the hospital chose to section HL although he did not meet the criteria and in December that year he was finally discharged by the hospital managers. When he got home he was in a terrible state says Mrs. E. The couple has a video showing the abuses to which HL had been subjected in the hospital: he looks half-starved with blackened toenails and scabs on his face. When he came home he just ate and slept for three weeks.Meanwhile the hospital trust supported by the Department of Health appealed to the House of Lords over the ruling. The Mental Health Act Commission suggested that 22000 people being detained informally would have to be detained formally under the Mental Health Act if the ruling were upheld. In 1998 the House of Lords overturned the ruling that HLs detention had been illegal. Mr. and Mrs. E decided to take the case to the European Court of Human Rights which in October 2004 ruled in HLs favour. As a result the government introduced the new Deprivation of Liberty Safeguards which came into force in April 2009.Source:http://www.equalityhumanrights.com/human-rights/our-human-rights-work/human-rights-inquiries/our-human-rights-inquiry/case-studies/the-bournewood-case/Layout of essay:1. GRANT PROPOSAL TOPIC
***My project idea is developing a grant that will assist with disruptive/unruly high school students. Once this proposal is developed and funded it will assist schools and their teachers with methods that will allow them to be more in control of their classrooms which will ultimately promote effectiveness and a higher graduation rate. Better classroom management techniques will be more appealing to stakeholders and other organizations because it will make education in the public school system more marketable when compared to private schools.
Assignment 2: Final Project: Writing the Grant Part 2: Methodology and Evaluation Plans
The methodology section is one of the core components of the grant. This section is concerned with addressing the question: “How will you carry out the project?” (Gitlin & Lyons, 2014, p. 92). For example, if your proposal is for a nonprofit-related project, you might identify the population served or indicate who will receive services. Depending on the grant type, you may also include the research sample and associated inclusion criteria. A research proposal may also include the research design strategy, assumptions, validity, reliability, statistical analysis, timeline, and evaluation plans. Finally, in this section you may address human resources requirements and materials or supplies needed to complete the project.
In recent years, evaluation plans have become increasingly important to prospective funders. Competition for grants is intense, and funders must feel confident that money will be well spent and that projects can be appropriately evaluated. This is an important consideration for the grant seeker as well. Your organization will also want to be assured that the time and effort put into a project proves that the objectives are met and if not, why.
For this Assignment:
Review the Final Project Guidelines provided in this module’s Learning Resources.
Carefully review your Gitlin & Lyons text, the University of Wisconsin-Extension resource, and the Community Tool Box resource as well as other Learning Resources that focus on methodology and evaluation. You may also find additional online information from reputable grant-related organizations, such as Foundation Center and individual private funders.
Remember that this content must demonstrate that it addresses funder interests as well as the grantee perspective.
In 3–5 pages, provide your Methodology and Evaluation Plans.
In developing the methodology for your project, keep these thoughts in mind:
A project should undertake only activities that will move it toward the objectives.
If any activities are not necessary for reaching the objectives, do not include them in the proposal.
The methodology includes the day-to-day, week-to-week, and/or month-to-month activities that will be carried out during the project period.
The Methodology includes:
Program description
Research design
Human subjects (sample)
Study validity and reliability
Assumptions and limitations
Time line
Analysis (and/or see Evaluation below)
Non-personnel resources
Facilities, supplies, technology, etc.
Any non-personnel resources that will appear in the proposal budget should be described
Personnel resources
Who will be assigned or hired?
Management plan (Work plan)
Who will be responsible for which activities
What is the timeline associated with activities (first quarter, second quarter, third quarter, fourth quarter, ongoing)?
How will the project be managed?
The Evaluation Plan includes:
Description of the specific criteria that will measure the success of the project
Explanation of data that will be collected
Verify if your grant application will require an institutional review board (IRB) approval.
Explanation of the data collection plans at the beginning, mid-program, and final evaluation
Evaluation instruments that will be used and an explanation of why these instruments were selected
Plans for writing the periodic reports to keep the funder updated

GRANT PROPOSAL TOPIC
***My project idea is developing a grant that will assist techers with disruptive/unruly high school students.
Discussion: Budget, Budget Narrative, & Sustainability Plan
The budget is a crucial piece in the logical progression of developing your proposal. Look back at the other key components—needs statement, objectives, goals, methodology, and evaluation—and consider how these pieces must be represented in and supported by the budget. One can see why a funding reviewer might begin by looking at a budget. It should fully, clearly, and succinctly depict the story of the project. The budget narrative expands upon the line items of a budget, offering further explanation of costs and needs, as appropriate.
A sustainability plan explains to funders how you will continue a project after funding has been exhausted. Not only does this plan demonstrate a proactive stance toward your project, but it also provides a funder assurance that its interests will be served beyond the project timeframe.
In preparation for this Discussion, consider the components of your project that you have determined to this point. Review this module’s Learning Resources that focus on the budget, budget narrative, and sustainability plan. Note: You may select one of the two budget formats for your Discussion post: the budget example on pp. 162–163 or use the Sample Budget Worksheet on p. 328 as a guide to develop your budget.
